Experiencia de momentos vividos por las familias de pacientes que estuvieron hospitalizados en unidad de cuidado intensivo

Abstract

El proceso de tener un familiar hospitalizado en una unidad de cuidado intensivo (UCI) es percibido por la familia como un tránsito de emociones y cambios a lo largo de ese determinado periodo de tiempo, y que se describe en diferentes momentos y contextos. La hospitalización y el proceso de recuperación y rehabilitación es lento, difícil y requiere adaptación a nuevas realidades que poco se han estudiado desde el punto de vista cualitativo. En ese sentido, los objetivos del presente estudio fueron describir la experiencia de los diferentes momentos vividos por las familias, describir las emociones por los que transita la familia de los pacientes posterior al alta y conocer el rol que asume la familia. Para llevar a cabo la investigación se empleó la metodología cualitativa de tipo teoría fundamentada, corriente constructivista de Katy Charmaz. La recolección de datos se llevó a cabo a través de entrevistas semiestructuradas y diarios escritos por 14 participantes, en dos momentos, al egreso de la UCI y tres meses posteriores a ello; tales participantes fueron familiares directos de pacientes, mayores de edad y tomadores de decisiones. Una vez realizado el análisis, se construyeron tres categorías conceptuales: viviendo emociones positivas, negativas y cambios físicos, creyendo en un ser superior y cambiando de rol en relación con el cuidado. Para concluir, se construyó un postulado teórico que permite conocer cómo describen las familias la experiencia de haber tenido un paciente hospitalizado en una unidad de cuidado intensivo. Estos resultados representan un aporte significativo al conocimiento de enfermería en la atención del paciente en estado crítico, teniendo en cuenta a la familia, quienes se convierten en cuidadores y pueden presentar alteraciones durante este proceso. Having a relative hospitalized in the Intensive Care Unit (ICU) means for the rest of the family a myriad of emotions and adjustments over a long period of time that involves a variety of circumstances. The path begins with the initial hospitalization and goes through a long and difficult road towards rehabilitation and home. This transition implies a constant adjustment to new realities that have not been extensively characterized in qualitative research. The main goals of these study are to describe the experience of families in different stages of the process; to describe the emotions families go through after discharge, and finally, to characterize the role of the family in the care of these patients. To conduct the research we used a qualitative method known as grounded theory. It is a constructivist approach by Katy Charmaz. Data were obtained through semi-structured interviews and diaries written by 14 subjects at two points in time: at discharge from the ICU and at 3 months after discharge. Subjects were older than 18 years old, they also were the decision maker for the patient and relative in first order. Once the analysis was performed, we developed 3 conceptual theories: experiencing positive and negative emotions as well as physical changes, belief in a divinity and assuming a role as a caregiver. To conclude, a proposed theory was developed that allows a better understanding of the experience of families during hospitalization of their loved one in the ICU. These results represent a significant advance for nurses and healthcare providers that can be applied to the counseling and orientation of families and caregivers
