Perfil de resistencia antimicrobiana en líquido peritoneal de pacientes hospitalizados por peritonitis en el Hospital General del Norte de Guayaquil Los Ceibos durante el período del 2017 a 2021.

Resumen
Las infecciones intraabdominales en su mayoría son consecuencia de una perforación o
debido a la inflamación de la pared intestinal, produciendo de esta manera una
peritonitis generalizada o en ocasiones colecciones localizadas como abscesos. El
cuadro clínico puede ser leve que inicia desde fiebre, anorexia, hasta progresar a un
cuadro severo de sepsis, con la consiguiente falla multiorgánica, choque séptico e
inclusive provocar la muerte.
El objetivo fue determinar el perfil de resistencia antimicrobiana en el líquido peritoneal
en el periodo 2017 al 2021. Método: Estudio observacional retrospectivo, transversal y
descriptivo, fueron estudiados todos los pacientes con diagnóstico de infección
intraabdominal y con cultivo de líquido peritoneal positivo. Resultados: Fueron
incluidos 82 pacientes, la edad más frecuente fue entre los 31 a 45 años, los hombres
fueron los más afectados con el 64 % de los casos. La bacteria más frecuentemen te
aislada fue Escherichia coli (47%), con una mortalidad por esta afección del 30 %. Se
encontró además que los antimicrobianos más sensibles fueron los carbapenémicos y
aminoglucósidos, mientras que los más resistentes fueron la ampicilina sulbactam y
ciprofloxacino. Intra-abdominal infections are mostly a consequence of a perforation or due to
inflammation of the intestinal wall, thus producing generalized peritonitis or sometimes
localized collections such as abscesses. The clinical picture can be mild that starts from
fever, anorexia, to progress to a severe picture of sepsis, with the consequent multiple
organ failure, septic shock and even cause death.
The objective was to determine the profile of antimicrobial resistance in the peritoneal
fluid in the period 2017 to 2021. Method: Retrospective, cross-sectional and descriptive
observational study, all patients with a diagnosis of intra-abdominal infection and
positive peritoneal fluid culture were studied. Results: 82 patients were included, the
most frequent age was between 31 to 45 years, men were the most affected with 64% of
cases. The most frequently isolated bacterium was Escherichia coli (47%), with a
mortality from this condition of 30%. It was also found that the most sensitive
antimicrobials were carbapenems and aminoglycosides, while the most resistant were
ampicillin, sulbactam and ciprofloxacin.
