Asociación de conductos no tratados y periodontitis apical en primeros molares superiores : análisis tomográfico.

Resumen
Introducción: El éxito de la terapia endodóntica radica en el cumplimiento de sus
objetivos principales: preparación biomecánica, desinfección y obturación de los
conductos radiculares en sus tres dimensiones. Es de suma importancia estar informado
sobre las variaciones anatómicas que tiene cada una de las piezas dentales. La
periodontitis apical es una respuesta inflamatoria en el periápice que frecuentemente se
la asocia a conductos no tratados. Objetivo: Determinar la asociación de conductos no
tratados y la periodontitis apical en primeros molares superiores. Materiales y Métodos:
Se realizó un estudio retrospectivo y transversal con un diseño descriptivo, analítico,
observacional y no experimental. Se obtuvieron 2000 tomografías de las cuales 100 fueron
seleccionadas según los criterios de inclusión y exclusión. Resultados: Existe una
prevalencia del 67% de la periodontitis apical asociada a conductos no tratados en
primeros molares superiores. El conducto no tratado que más influencia tiene es el MV2
en un 51%. La configuración anatómica Tipo I según Vertucci es la más común en la raíz
mesiovestibular en un 42%. En el 64% de las tomografías se observó que, no existe
asociación de conductos no tratados y otras entidades patológicas. El mejor volumen de
adquisición para la visualización de los conductos radiculares es el de 6×6cm.
Conclusiones: La probabilidad de encontrar 2 conductos en la raíz mesiovestibular de
los primeros molares superiores es bastante elevada. Un volumen de adquisición pequeño
es el mejor para el análisis anatómico. Introduction: The success of endodontic therapy lies in the fulfillment of its main
objectives: biomechanical preparation, disinfection and obturation of root canals in their
three dimensions. It is of the utmost importance to be informed about the anatomical
variations that each of the dental pieces has. Apical periodontitis is an inflammatory
response at the periapex that is frequently associated with untreated canals. Objective:
To determine the association of untreated canals and apical periodontitis in upper first
molars. Materials and Methods: A retrospective and cross-sectional study was carried
out with a descriptive, analytical, observational and non-experimental design. Two
thousand scans were obtained, of which 100 were selected according to the inclusion and
exclusion criteria. Results: There is a 67% prevalence of apical periodontitis associated
with missed canals in maxillary first molars. The missed canal that has the most influence
is MV2 in 51%. The anatomical configuration Type I according to Vertucci is the most
common in the mesiobuccal root in 42%. In 64% of the CT scans, it was observed that
there is no association of missed canals and other pathological entities. The best
acquisition volume for visualization of root canals is 6×6cm. Conclusions: The probability
of finding 2 canals in the mesiobuccal root of the maxillary first molars is quite high. A small
acquisition volume is the best for anatomical analysis.
