Prevalencia de Hipotiroidismo e Hipertiroidismo en pacientes con Melasma atendidos en el Servicio de Dermatología del Hospital General del Norte de Guayaquil (HGNGC) IESS Los Ceibos durante el periodo desde abril del 2017 hasta marzo del 2020.

Resumen
El melasma es un trastorno dermatológico definido por
hiperpigmentación y síntesis de la melanina en la piel, la prevalencia en
América Latina es del 8.8%, dentro de las distintas causas que se han
investigado hay evidencia científica que existe una relación con el perfil
hormonal tiroideo. En el presente estudio transversal, descriptivo,
observacional, retrospectivo se extrajeron datos de las historias clínicas del
sistema informático AS400 proporcionado por el HGNG IESS Los Ceibos
durante el periodo comprendido entre abril del 2017 y marzo del 2020; en
donde se determinó una prevalencia de 31.93% en una población de 332
pacientes en donde estadísticamente los pacientes hipotiroideos son del
97.2%, con una media de edad de 57.5 años, una media del IMC de 28.13
kg/m2; a su vez, el 49.1% de la población de estudio tenía un fototipo de piel
III, con afectación centro facial importante en 32.1% de los pacientes de
estudio. Los valores de TSH presentaron un promedio de 8.16 mUl/L y un
promedio de FT4 de 1.41 ng/dl, encasillándose sobre estos promedios un
diagnóstico de hipotiroidismo subclínico. La prevalencia de hipotiroidismo e
hipertiroidismo en pacientes con melasma fue mayor al propuesto en la
hipótesis, alcanzando casi un tercio de la población de estudio, donde casi su
totalidad fueron pacientes con hipotiroidismo. Melasma is a dermatological disorder defined by hyperpigmentation
and melanin synthesis in the skin, the prevalence in Latin America is 8.8%,
among the various causes that have been investigated there is scientific
evidence that there is a relationship with the thyroid hormonal profile.
In the present cross-sectional, descriptive, observational, retrospective study,
data were extracted from the medical records of the AS400 computer system
provided by the HGNG IESS Los Ceibos during the period between April
2017 and March 2020; where a prevalence of 31. 93% in a population of 332
patients where statistically hypothyroid patients are 97.2%, with a mean age of
57.5 years, a mean BMI of 28.13 kg/m2; in turn, 49.1% of the study
population had skin phototype III, with significant facial center involvement in
32.1% of the study patients. TSH values presented an average of 8.16 mUl/L
and an average FT4 of 1.41 ng/dl, pigeonholing on these averages a
diagnosis of subclinical hypothyroidism. The prevalence of hypothyroidism
and hyperthyroidism in patients with melasma was higher than proposed in
the hypothesis, reaching almost a third of the study population, where almost
all of them were patients with hypothyroidism.
