Relación de la enfermedad periodontal en la progresión de la patología ocasionada por el Sars-Cov 2 : una revisión sistemática.

Resumen
Objetivo:Plantear la posible relación entre la enfermedad periodontal y la
progresión de la patologia ocasionada por el SARs-CoV 2, basado en tres
teorias:Teoria de replicación viral,Teoria endotelial,Teoria inflamatoria .
Materiales y métodos: Revisión de la literatura empleando metabuscadores
PubMed, Cochrane Library y Google Scholar cuyo resultado dio una muestra
de 121 artículos con los que se analizó las tres teorías
Resultados: De la primera teoría 86% de los autores concordo quea mayor
carga viral peor es el pronóstico para el paciente, el 95% está de acuerdo con
que hay ACE2 en las células endoteliales, 88% confirman un aumento de
vasos sanguíneos (células endoteliales) en la enfermedad periodontal y de 15
autores 12 mencionan un aumento de neutrófilos (presentan CD147) en la
enfermedad periodontal. En la teoría endotelial se analizó el impacto de la
unión del virus al ACE2 dando como resultado inflamación (21%) , desbalance
del RAS (18%), regulación negativa del ACE2 (16%), entre otros efectos. A
su vez 10 artículos denominaban a la periodontitis como enfermedad
endotelial. En la teoría de la inflamación se estudió los biomarcadores
inflamatorios elevados a nivel sistémico en pacientes periodontalmente
comprometidos dando como resultado:TNF (23%),IL 6(20%),IL 1 (20%)
,CRP (26%) ,IL 8 (6%),IL 10 (3%) y IFN-γ (3%)
Conclusión: Hay numerosos factores presentes en las personas que padecen
de periodontitis que pueden llegar a contribuir en la progresión de la
enfermedad COVID 19 , considerando que se puede ver incrementada la
cantidad de carga viral inicial, los efectos endoteliales (endotelitis, agregación
plaquetaria , entre otros), y la inflamación a nivel sistémico especialmente los
niveles de IL-6 y CRP. Todo esto da hincapié a que la enfermedad periodontal
se debe considerar como una comorbilidad que contribuye a la progresión de
la enfermedad COVID 19. Objective: Propose the posible relationship between the periodontal disease and the
COVID 19, based on three theories :viral replication theory , Endothelial Theory ,
Inflammation theory .
Materiales y métodos: Literature review using the metasearch engines PubMed, Cochrane
Library and Google Scholar that resulted in 121 articles with which the three were
analyzed,
Results: Of the first theory 86% of the authors agree that more viral load aggravates the
progression of the COVID 19 ,95% says that there is ACE2 in the endothelial cells, 88%
confirmed that there’s an increase of blood vessels during the periodontal disease also out
of 15 authors 12 mention that there’s and increase of neutrophils (CD147) . The endothelial
theory took into account the impact of the union of the virus and the ACE2 that resulted in
inflammation (21%) , unbalance of the RAS (18%), negative regulation of the ACE2
(16%). At the same time 10 articles said that the periodontal disease is and endothelial.
