Global mortality and annual functional capacity of patients after hip fracture surgery

Resumen
Background: Low-energy hip fracture is the main complication of osteoporosis regarding mortality, disability and economic burden. To our knowledge, this would be the first study in a Ministry of Health hospital that evaluates mortality and functional capacity in patients with low-energy hip fracture who underwent surgery. Objectives: Determine mortality in patients with a low-energy hip fracture who underwent surgery. Methods: We performed an observational retrospective single-cohort study during the 2016-2019 period, formed by collecting operative notes of patients with hip fracture that underwent surgery. Patients under the age of 50 years and those with a secondary osteoporosis diagnosis were excluded. Results: We included 163 patients with a median age of 78.9 years ± 10.5. Mortality during the first 12 months after surgery was 13.5% and 30.67% at the end of the follow-up (mean of 28.91 months). We found higher mortality in men (38.78%), compared with women (27.19%). Likewise, 52.48% of patients were limited to perform vocational and avocational activities after the first year of surgery and 68.71% did not receive osteoporosis treatment after surgery. Conclusions: Low-energy hip fracture during the first 12 months after surgery is associated with a 13.5% mortality, higher than in general population, and a low functional capacity. Antecedentes: La fractura de cadera por trauma de bajo impacto es la principal complicación de osteoporosis en cuanto a mortalidad, discapacidad y costos sanitarios. Este sería el primer estudio que evalúa mortalidad y capacidad funcional en pacientes posoperados por fractura de cadera por trauma de bajo impacto en un hospital del Ministerio de Salud. Objetivo: Determinar la mortalidad en pacientes operados por fractura de cadera por trauma de bajo impacto. Material y métodos: Es un estudio observacional de tipo cohorte única retrospectiva correspondiente al periodo 2016-2019, conformada mediante la revisión de informes operatorios de pacientes operados por fractura de cadera. Se excluyeron pacientes menores de 50 años y con diagnóstico de osteoporosis secundaria. Resultados: Se incluyeron 163 pacientes con una edad media de 78,9 años ± 10,5. La mortalidad en los primeros 12 meses posteriores a la cirugía fue de 13,5% y al final del seguimiento (media de 28,91 meses) de 30,67%. La mortalidad fue mayor en hombres (38,78%), comparada con las mujeres (27,19%). El 52,48% de pacientes tuvo limitaciones para realizar actividades vocacionales y no vocacionales luego del primer año de cirugía y el 68,71% no recibió ningún tratamiento posterior a la operación. Conclusiones: La fractura de cadera por trauma de bajo impacto en los primeros 12 meses tras la operación se asocia a una mortalidad de 13,5%, mayor a la de la población general, y a una baja capacidad funcional.
