Efecto Anticonvulsivante de la Asociación de Fenitoína e Inhibidores de la Glicoproteína-P (Elacridar y Tariquidar) en el Estatus Epiléptico Inducido por Pilocarpina en Ratas Sprague-Dawley

Resumen
La epilepsia es el cuarto trastorno neurológico más común en el mundo, en la
actualidad el 30% de los pacientes con epilepsia presentan resistencia a la terapia
farmacológica, la cual es mayor en fármacos de primera línea como fenitoína; esto se
podría deber a la acción de la Glicoproteína-P, que expulsa las sustancias xenobióticas
al exterior de la célula, impidiendo así que el antiepiléptico alcance la concentración
necesaria para suprimir las crisis epilépticas. Elacridar y tariquidar son fármacos
inhibidores de la Glicoproteína-P de tercera generación, que se caracterizan por una
acción rápida y específica. Es así que el presente estudio tuvo como objetivo evaluar
el efecto anticonvulsivante de la asociación de fenitoína e inhibidores de la
Glicoproteína-P (elacridar y tariquidar) en el estatus epilépticus en un modelo de
epilepsia inducido por pilocarpina en ratas Sprague-Dawley.
Se trabajó con 123 animales de experimentación los cuales fueron divididos en 23
grupos: 5 Grupos (n=3 c/u) fueron utilizados para la determinación de la dosis mínima
efectiva de fenitoína, la cual fue utilizada en todos los tratamientos de asociación con
los inhibidores de la Glicoproteína-P; Grupo control diazepam 4mg/Kg (n=6), Grupo
vehículo (n=6), Grupo positivo al cual se le administró la dosis mínima efectiva
determinada de fenitoína (n=6), Grupos de Tratamiento 2, 3, 4, 5 y 6 a los cuales se
les administro fenitoína con elacridar 0.5, 1, 1.5, 2 y 3 mg/Kg (n=6 c/u)
respectivamente, Grupos de Tratamiento 7, 8, 9, 10 y 11 a los cuales se les administro
fenitoína con tariquidar 0.5, 1, 1.5, 2 y 3 mg/Kg (n=6 c/u) respectivamente y Grupos
de Tratamiento 12, 13, 14, 15 y 16 los cuales recibieron fenitoína con elacridar –
tariquidar a las dosis de 0.5, 1, 1.5, 2 y 3 mg/Kg (de cada inhibidor) (n=6 c/u)
respectivamente . Todos los tratamientos fueron administrados por vía intravenosa tres
horas después de iniciado el estatus epilépticus y evaluados por un score de respuesta
terapéutica a los 15, 30 y 60 minutos post-administración.
En el presente estudio se determinó como dosis mínima efectiva de fenitoína la dosis
de 15 mg/Kg. A los 30 minutos de evaluación, todas las asociaciones mostraron
porcentajes de efectividad superiores a los observados a los 15 y 60 minutos. Los
tratamientos de asociación de fenitoína con elacridar 3 mg/Kg (110%), tariquidar 1.5
mg/Kg (102%) y 2 mg/Kg (99%); elacridar - tariquidar 1 mg/Kg (102%), 1.5 mg/Kg
(102%), 2 mg/Kg (103%) y 3 mg/Kg (102%), presentan una efectividad
estadísticamente superior a la obtenida por fenitoína 15 mg/Kg (83%). La asociación
de fenitoína 15mg/Kg con un solo inhibidor, elacridar 3mg/Kg (110%) presenta la
mayor efectividad; y de los tratamientos de asociación con ambos inhibidores las dosis
de elacridar – tariquidar 1, 1.5, 2 y 3 mg/Kg presentan un efecto anticonvulsivante
similar (102%, 102%, 103% y 102% respectivamente).
Se concluye que la asociación de fenitoína con ambos inhibidores de la Glicoproteína-
P presenta un mejor efecto anticonvulsivante, siendo la asociación de fenitoína 15
mg/Kg con elacridar – tariquidar en dosis de 1 mg/Kg cada una, la mejor alternativa
para el tratamiento del estatus epilépticus en ratas Sprague-Dawley.
