Empyema necessitatis is a rare disease that is not really common nowadays reason why there is not much information about it. This article describes the clinical case of an 87- year old patient who presented a visible thoracic mass and painful on palpation and when coughing. The study of Computed Axial Tomography (CT) with contrast showed lung expansion comparatively with less expansion on the left lung. This also revealed a hypodense collection of approximately 180cc of volume which extended through the chest wall. A posterolateral left thoracotomy with organized abscess drainage of left chest was made. El Empiema necessitatis es una patología muy poco frecuente en estos tiempos, razón por la cual no se tiene registrada mucha información al respecto. Se presenta el caso clínico de un paciente masculino de 87 años de edad que presenta una masa visible a nivel de hemitórax izquierdo, dolorosa a la palpación y al provocar la tos. El estudio del Tomografía Axial Computarizada (TC) con medio de contraste evidencia pulmones expandidos comparativamente con menor expansión del pulmón izquierdo, se observa colección hipodensa de aproximadamente 180cc de volumen, que se extiende a los tejidos de la pared torácica. Se realiza toracotomía posterolateral izquierda con drenaje del absceso organizado de la pared torácica izquierda.
