Thrombin generation and phenotypic correlation in haemophilia A

A new dissipative model is proposed for the small component of a mixture of a small and large component of gases, where the large component is treated as an ideal gas. The thermodynamic characteristics of the gas are derived through a statistical mechanical approach. The model requires for the small component to have a big dimension and its number of particles to be smaller than the large one. Using the associated partition function, the internal energy and the equation of state are calculated. The internal energy does not suffer any deviation from that of the two component ideal gas, but the equation of state deviates from that of the ideal gas for large values of the parameter which characterizes the dissipative model. Soltero, J.F.A.",,"2006",,"The synthesis of nanostructured poly(N-isopropylacrylamide) (polyNIPA) hydrogels by a two-stage polymerization process is reported here. The process involves the synthesis of slightly crosslinked polyNIPA nanoparticles by inverse (w/o) microemulsion polymerization; then, these particles are dried, cleaned and dispersed in an aqueous solution of NIPA and a crosslinking agent (N, N-methylene-bis-acrylamide or NMBA) and polymerized to produce the nanostructured hydrogels. Their swelling and de-swelling kinetics, volume phase transition temperatures (TVPT) and mechanical properties at the equilibrium swollen state are investigated as a function of the weight ratio of polyNIPA particles to monomer (NIPA). The nanostructured gels exhibit larger equilibrium water uptake, faster swelling and de-swelling rates and similar TVPT than those of the conventional ones; moreover, the elastic and Young moduli are larger than those of the conventional hydrogels at similar swelling ratios. Laffan, M.A.",,"2005",,"The clinical phenotype of patients with haemophilia A (HA) often differs between individuals with the same factor VIII (FVIII) gene defect (e.g. within the same family) or the same coagulant activity of FVIII (FVIII:C). We proposed that because the thrombin generation assay in platelet-poor plasma of HA patients provides more information [peak thrombin concentration, endogenous thrombin potential (ETP), rate of thrombin generation and lag-time] than a clot-based FVIII assay it might provide insight into these differences. We therefore investigated the relation between the results of the thrombin generation assay and the clinical severity in nine families with HA (23 patients with different phenotypes). We also examined the contribution of prothrombotic risk factors: (FV Leiden G1691A and prothrombin G20210A), the coagulant activity of FVIII and tissue factor (5?UTR) polymorphisms. Our data detect marked differences between individuals but these did not correlate with the reported clinical phenotype. These differences were also reflected in a marked difference in response to the therapeutic amounts of FVIII. This might account for differences in amounts of treatment consumption. Reduced peak and possibly rate of thrombin generation, rather than FVIII:C or ETP appear to represent the critical defects in FVIII-deficient plasma. We suggest that the analysis of parameters in thrombin generation is a useful tool to detect bleeding tendency in HA but not to predict the modulation of the haemorrhagic tendency in patients within families. However the presence of the other factors such as vessel wall components, protein C and platelets might need to be incorporated into this system. " 2005 Blackwell Publishing Ltd.
