ABSTRACT: Allergic diseases affect 20% of the world population. Allergic rhinitis (AR) covers alone or associated with asthma, more than 80% of the allergic diseases. Allergies are influenced by genetic factors and share a common physio-pathologic mechanism of hypersensitivity type I. To reduce the cost of AR and to improve the quality of life from the patients is a world concern. International initiatives like ARIA (Allergic Rhinitis and its Impact on Asthma), classify and propose a pharmacologic treatment for AR. However, specific antigen immunotherapy is considered one of the best treatments for AR, since many studies have demonstrated that specific antigen immunotherapy may alter the natural history of the disease by modifying the immunological response, increasing the function of clones of helper T type 1 lymphocytes (Th1) and inhibiting the activity of helper T type 2 lymphocytes (LTh2). It is believed that the dialyzable extract of leukocytes or transfer factor (FT) also has a stimulatory effect for Th1 and an inhibitory effect for Th2. Therefore, we have proposed that FT and SLITAE may have a synergic effect. In order to test this hypothesis, we studied 54 AR patients (50% women and 50% men, with a mean age of 15 + 10 and a range age between 5 and 40 years) with positive skin Tests and/or serum specific IgE. All patients were evaluated both at the beginning and at the end of 3 months treatment. Twelve healthy individuals that served as controls were evaluated once in the same manner. Informed consent was signed either by the patients or by the parents when the patient was under age. Patients were randomly distributed in 2 groups. All patients received specific antigen sublingual Immunotherapy (SLITAE) but group 1 additionally received orally a monthly unit of transfer factor and group 2 received placebo instead of FT (Transferon*), administered in the same way. A complete cell blood count was performed. Immunoglobulin (IgG, IgM, IgA and IgE), lymphocytes (CD3+, CD4+, and CD8+) and Interleukins (IL-2, IL-4, IL-5, IL-10, TNF and IFN) were measured. In addition, nasal symptoms were classified on a scale from 0 to 3 at the beginning and at the end of treatment. In addition, the patients also evaluated their own global improvement by means of a visual analog Likert-type scale. Results: Fifty four AR patients were randomly placed in 2 groups and treated with SLITAE and TF or SLITAE and placebo. Indoor antigens (mites, house dust and cockroach) were the main cause of allergy. AR patients were analyzed at the beginning and at the end of 3 months treatment. Twelve non-allergic subjects were evaluated once and served as healthy controls for comparison purposes. Allergic patients had more eosinophiles and IgE than the healthy controls (p<0.05). The number of CD8+ lymphocytes was slightly reduced in group 2 after treatment (p<0.05), whereas the amount of IL-4 and IFNwere increased in both groups (p<0.005) and the amount of IL-10 was significantly increased in group 1 (p<0.01) after treatment. Both groups of patients showed a significant improvement of symptoms as evaluated by themselves on a Likert-type scale. RESUMEN: Las enfermedades alérgicas afectan al 20 % de la población mundial. La rinitis alérgica (RA) sola o asociada al asma cubre mas del 80% de las enfermedades alérgicas. Las alergias tienen una influencia genética y un mecanismo fisiopatológico en común de hipersensibilidad tipo I. Para reducir los costos y mejorar la calidad de vida se han propuesto iniciativas mundiales como ARIA (Allergic Rhinitis and its Impact on Asthma), las cuales proponen las clasificaciones y el manejo farmacológico de la RA y el asma. La inmunoterapia antígeno específica por vía sublingual (SLITAE) es uno de los tratamientos más eficaces en el manejo de la RA, la cual puede modificar la historia natural de las enfermedades alérgicas, induciendo cambios en la respuesta inmunológica como por ejemplo el aumento en la función de clonas de linfocitos T cooperadores de tipo 1 (Th1) y la inhibición de la actividad de los linfocitos T cooperadores de tipo 2 (Th2). Se ha observado tambien que el extracto dializable de leucocitos o factor de transferencia (FT) (Transferon*) puede tener un efecto estimulante de linfocitos Th1 e inhibitorio de Th2.Es por esto que propusimos la hipótesis de que el FT y la SLITAE pueden tener un efecto sinérgico en el manejo de pacientes con RA. Para comprobar esta hipótesis estudiamos 54 pacientes (50%hombres y 50% mujeres) con RA confirmada por historia clínica y pruebas cutáneas y/o IgE específica contra aeroalergenos , con edades entre 5 y 40 años, evaluando a cada uno al inicio y a los 3 meses de haber iniciado el tratamiento. Incluimos 12 personas sanas como grupo testigo haciendo evaluación solo al inicio del estudio. Los pacientes o sus padres en el caso de los menores de edad firmaron consentimiento informado antes de iniciar el estudio. Los pacientes se distribuyeron al azar en 2 grupos. Ambos grupos recibieron SLITAE. El grupo 1 recibió tambien Ft (transferon*) 1 unidad mensual por 3 meses. Al grupo 2 se le dio un placebo en la misma presentación y misma vía. En todos los casos se realizaron conteos celulares sanguíneos, CD3+, CD4+ y CD8+, medición de interleucinas (IL-2, IL-4, IL-5, IL-10, TNFe IFN). Los síntomas nasales se calificaron por medio de una escala tipo Likert y por medio deTSSQ4 (Test symptom severity questionnaire) Resultados: Se estudió a 54 pacientes con RA y 12 personas sanas. Los pacientes alérgicos tenían más eosinófilos que las personas sanas (p< 0.05), la cantidad de linfocitos CD8+ se redujo en el grupo 2 posterior al tratamiento (p< 0.05). La IL-4 y el IFN aumentaron en ambos grupos posterior al esquema terapéutico (p< 0.005), la IL-10 se incrementó posterior al tratamiento en el grupo 1 (p<0.01). Ambos grupos de pacientes demostraron mejoría importante de los síntomas evaluados tanto por TSSQ4 como por la escala tipo Likert.
