Older people, even those with good health, grow weak-slowly because of the
physiological changes that over time, brings limitations to the functions of the
body, making them increasingly predisposed to addiction to the achievement of
self-care, to loss of autonomy and quality of life. They become also more
susceptible to disease and, as a result, it comes to hospitalization. The elderly
after hospital discharge is fragile, has a high degree of dependence, weakness,
fear of falls, injuries, postural instability among other more. Much of the elderly
living alone and conducts its activities without the need for monitoring, but a
substantial part, needs care and permanent guidelines. In this sense the staff of
the Family Health Strategy team (FHT) of Orizânia-Mg prepared a contingency
plan for the systematic multidisciplinary home care to the elderly frail, after
hospital discharge. To accomplish this it was necessary to face several critical
nodes that interfered in the work process. Such measures fills big gap of
assistance to the population of dependent elderly providing better quality of life
to the patient or even minimizing suffering and organizing family involvement in
care and team work process.
