Resumen
The city of São José da Tapera has a land area of 495 km² with a population of 30,140 (thirty thousand one hundred forty) people. The health team to which I belong, meets the population belonging to the Health Program New Farm Family and has a population of 2,665 inhabitants and 637 families. The hypertension since 2000 has been one of the main factors of morbidity and the estimated worldwide prevalence shows an increase of around 26% should go to 29% in 2025, considering only the population growth and the age composition. Globalization, consumerism, the need for early pleasures and immediate responses contribute to the onset of hypertension as a social issue. In our health, the main cause of death is related to vascular disease morbidity from hypertension. This study aimed to draw up a contingency plan, propose actions to increase patients' knowledge about the disease and intervene in risk factors for hypertension control. For basement study was done a literature search in the Scientific Electronic Library Online (SciELO) on the subject and were even used manuals and Health Ministry documents. The plan seeks to offer a proposal of eating habits and style of life appropriate, and a better patient care and monitoring of risk factors for high blood pressure and improve the quality of health care provided by health care team for patients with Hypertension.
