Objetivo: Determinar si el score de RIPASA tiene mayor valor diagnóstico que
el score de Alvarado modificado para Apendicitis aguda en el Hospital Víctor
Lazarte Echegaray, en el periodo 2018.
Material y método: estudio retrospectivo, observacional, de pruebas
diagnósticas, donde se evaluó 268 historias clínicas de pacientes con
diagnóstico preoperatorio de apendicitis aguda.
Resultados: El test de RIPASA, tuvo una Sensibilidad de 23.3%, Especificidad
94.7%, Valor predictivo positivo 98.3% y Valor predictivo negativo 8.6%. El test
de Alvarado modificado, tuvo una Sensibilidad de 51.0%, Especificidad 63.2%,
Valor predictivo positivo 94.8% y Valor predictivo negativo 9.0%. Al comparar
ambos scores se observa una diferencia estadística altamente significativa
(p<0.01) con respecto a la Sensibilidad, siendo más alta en el score de Alvarado
modificado. La caracterización del grupo de pacientes según edad, muestra que
el 66.0% se ubicaron entre 20 – 49 años; y solamente el 4.9% tuvieron de 70 a
más años. La edad media y la desviación estándar fueron de 35.7 y 16.2 años
respectivamente. Respecto al sexo, el 57.8% fueron de sexo masculino y el
42.2% de sexo femenino.
Conclusiones: El score de Alvarado modificado mostró mayor valor diagnóstico
que el score de RIPASA en el diagnóstico de apendicitis aguda. Objective: To determine if the RIPASA score has a higher diagnostic value than
the modified Alvarado score for acute appendicitis at the Victor Lazarte
Echegaray hospital, in the 2018 period.
Material and method: retrospective, observational study of diagnostic tests,
where 268 medical records of patients with preoperative diagnosis of acute
appendicitis were evaluated.
Results: The RIPASA test had a Sensitivity of 23.3, Specificity 94.7%, Positive
Predictive Value 98.3% and Negative Predictive Value 8.6%. The modified
Alvarado test, had a Sensitivity of 51.0%, Specificity 63.2%, Positive Predictive
Value 94.8% and Negative Predictive Value 9.0%. When comparing both scores
a highly significant statistical difference (p <0.01) with respect to Sensitivity is
observed, being higher in the modified Alvarado score. The characterization of
the group of patients according to age, shows that 66.0% were between 20-49
years; and only 4.9% had 70 or older. The mean age and standard deviation were
35.7 and 16.2 years respectively. Regarding sex, 57.8% were male and 42.2%
female.
Conclusions: The modified Alvarado score showed a higher diagnostic value
than the RIPASA score in the diagnosis of acute appendicitis.
