Factores de riesgo para hernia incisional umbilical en pacientes sometidos a colecistectomia laparoscopica

OBJETIVO: Determinar los factores de riesgo para desarrollar hernia incisional
umbilical en pacientes sometidos a colecistectomía laparoscópica.
MATERIAL Y MÉTODOS: Se realizó un estudio observacional, analítico,
retrospectivo de casos y controles. Se incluyó 76 pacientes post colecistectomía
laparoscópica atendidos en el Hospital Militar Central, dividido en 02 grupos: un
grupo casos conformado por 19 pacientes que desarrollaron hernia incisional
umbilical y un grupo control con 56 pacientes que no presentaron hernia
incisional umbilical, durante el periodo 2013 al 2018. Se evaluaron los siguientes
factores de riesgo: sexo, edad, obesidad, infección de sitio operatorio, ampliación
de orificio umbilical, tipo de abordaje, diabetes mellitus e hipertensión arterial.
RESULTADOS: Se encontró que el sexo femenino (p= 0,791) y un OR:0,87, la
ampliación del orifico umbilical (p= 0,000) y un OR:55,47 y la diabetes mellitus
tipo2 (p= 0,000) y un OR:10,50 son factores de riesgo estadísticamente
significativos (IC 95%, 1.941 – 6.243). Además, se demostró que la edad mayor
o igual a 70 años, la obesidad, infección de sitio operatorio, tipo de abordaje e
hipertensión arterial no son factores de riesgo para el desarrollo de hernia
incisional umbilical.
CONCLUSIONES: Se concluye que sexo femenino, la ampliación del orifico
umbilical y la diabetes mellitus tipo2 son factores de riesgo para el desarrollo de
hernia incisional umbilical. OBJECTIVE: Det the risk factors to develop umbilical incisional hernia in patients
undergoing laparoscopic cholecystectomy.
MATERIAL AND METHODS: An observational, analytical, retrospective study of
cases and controls was carried out. We included 76 patients after laparoscopic
cholecystectomy treated at the Central Military Hospital, divided into 02 groups:
a group consisting of 19 patients who developed umbilical incisional hernia and
a control group with 56 patients did not present umbilical incisional hernia, during
the period 2013 to 2018 The following risk factors were evaluated for all
participants: sex, age, obesity, infection of the operative site, enlargement of the
umbilical orifice, type of approach, diabetes mellitus and arterial hypertension.
RESULTS: It was found that the female sex (p = 0.791) and an OR: 0.87, the
enlargement of the umbilical orifice (p = 0.000) and an OR: 55.47 and the
diabetes mellitus type2 (p = 0.000) and a OR: 10.50 are statistically significant
risk factors (95% CI, 1941 - 6.243). In addition, it was demonstrated that age
greater than or equal to 70 years, obesity, operative site infection, type of
approach and arterial hypertension are not risk factors for the development of
umbilical incisional hernia.
CONCLUSIONS: We conclude that female sex, umbilical orifice enlargement and
type 2 diabetes mellitus are risk factors for the development of umbilical incisional
hernia.
