Resulta interesante el entendimiento del desarrollo y erupción de los terceros molares, ya que por su compleja ubicación anatómica y su erupción tardía en la cavidad bucal representan el mayor número de accidentes infecciosos, quísticos y tumorales. Se considera que el 20% de las sombras radiotransparentes asociadas a la corona de un tercer molar incluido se puede corresponder con un Quiste Dentígero. La evolución de esta patología es silente y suele ser diagnosticada de manera incidental en exámenes radiográficos o cuando se producen afectaciones importantes en el complejo maxilofacial, como fracturas patológicas, desplazamientos dentarios, tumefacción facial, infecciones sobreañadidas y la transformación ameloblástica del epitelio quístico. Debido al riesgo potencial de desarrollo quístico y a su alta frecuencia se plantea como objetivo principal: establecer parámetros para la detección y diagnóstico de sacos foliculares asociados a terceros molares incluidos con características de desarrollo quístico y riesgo de transformación ameloblástica. El estudio se basó en el análisis de la literatura científica, fundamentada en la revisión de casos clínicos, artículos de metaanálisis y libros de referencia en el área de la patología oral y maxilofacial. Se concluye que los sacos foliculares de terceros molares incluidos representan un elevado riesgo potencial de desarrollo quístico y a su vez el quiste dentígero presenta de un 5 al 7% de desarrollo neoplásico dentro del cual el 33% corresponde a la transformación ameloblástica.
ABSTRACT
It is interesting to understand the development and eruption of third molars since due to their complex anatomical location and their late eruption in the oral cavity, they represent the largest number of infectious, cystic, and tumoral accidents. It is considered that 20% of the radiolucent shadows associated with the crown of an impacted third molar may correspond to a Dentigerous Cyst. The evolution of this pathology is silent and it is usually diagnosed incidentally in radiographic examinations or when significant affectations occur in the maxillofacial complex, such as pathological fractures, dental displacements, facial swelling, superimposed infections, and ameloblastic transformation of the cystic epithelium. Due to the potential risk of cystic development and its high frequency, the main objective is to establish parameters for the detection and diagnosis of follicular sacs associated with included third molars with characteristics of cystic development and risk of ameloblastic transformation. The study was based on the analysis of the scientific literature, based on the review of clinical cases, meta-analysis articles, and reference books in the area of oral and maxillofacial pathology. It is concluded that the follicular sacs of included third molars represent a high potential risk of cystic development and in turn, the dentigerous cyst presents 5 to 7% of neoplastic development within which 33% corresponds to ameloblastic transformation.
