Factores asociados a sepsis por implantofix en pacientes pediátricos con leucemia linfoblástica aguda

Introducción: El manejo primario de la Leucemia Linfoblástica Aguda es
por medio de Quimioterapias, que son medicamentos que actúan a nivel
de las células linfocitarias malignas de la médula ósea, destruyéndolas o,
impidiendo que se continúen replicando, permitiendo a la médula ósea
trabajar nuevamente con normalidad. En las salas de Oncología pediátrica,
son muy comunes la utilización de vías venosas centrales totalmente
implantables las cuales llevan a cabo un manejo más sencillo y práctico
durante el tratamiento de la leucemia. Sin embargo, el uso de estos
dispositivos es también el origen de un proceso infeccioso, si no es
controlado de forma oportuna y precoz.
Materiales y Metodología: El presente estudio es de enfoque mixto, de
corte transversal, y de tipo retrospectivo. La información fue obtenida a
partir de la revisión de Historias Clínicas de pacientes con diagnóstico de
Leucemia Linfoblástica Aguda del Hospital Dr. Francisco de Icaza
Bustamante.
Objetivo: Establecer los factores asociados a sepsis por Implantofix en
pacientes pediátricos de 1 a 12 años con Leucemia Linfoblástica Aguda del
Hospital Dr. Francisco de Icaza Bustamante en el periodo 2019 – 2021.
Resultados: Se determinó que los factores que más contribuyen a la
aparición de sepsis asociada al Implantofix son la Neutropenia, la
utilización del Implantofix por más de 6 días intrahospitalariamente y la
administración de antibioprofilaxis después de 24 horas de iniciado el
cuadro. De igual forma, es más frecuente el desarrollo de este cuadro
cuando el paciente se encuentra en la fase de Inducción de su tratamiento
y la estancia prolongada, los cuales dejan al paciente susceptible a
desarrollar nuevos cuadros infecciosos Introduction: The primary management of Acute Lymphoblastic Leukemia
is through chemotherapies, which are drugs that act at the level of malignant
lymphocyte cells of the bone marrow, destroying them or, preventing them
from continuing to replicate, allowing the bone marrow to work normally
again. In pediatric oncology wards, the use of fully implantable central
venous pathways is very common which carry out a simpler and more
practical management during the treatment of leukemia. However, the use
of these devices is also the origin of an infectious process, if it is not
controlled in a timely and early manner.
Materials and Methodology: The present study is of mixed approach,
cross-sectional, and retrospective type. The information was obtained from
the review of Clinical Records of patients diagnosed with Acute
Lymphoblastic Leukemia of the Dr. Francisco de Icaza Bustamante
Hospital.
Objective: To establish the factors associated with sepsis by Implantofix in
pediatric patients aged 1 to 12 years with Acute Lymphoblastic Leukemia
of the Dr. Francisco de Icaza Bustamante Hospital in the period 2019 –
2021.
Results: It was determined that the factors that contribute most to the
appearance of sepsis associated with Implantofix are Neutropenia, the use
of Implantofix for more than 6 days in hospital and the administration of
antibioprophylaxis after 24 hours of starting the frame. Similarly, the
development of this picture is more frequent when the patient is in the
Induction phase of his treatment and the prolonged stay which leave the
patient susceptible to developing new infectious pictures.
