Capacidad predictiva de mal pronóstico de la mamografía y ecografía en carcinoma mamario triple negativo

Antecedentes: Establecer el pronóstico pretratamiento en carcinoma de mama triple negativo (CMTN) permite dirigir el manejo terapéutico. La resonancia magnética cumple este propósito. Sin embargo, los costos asociados a su desarrollo y no estar siempre disponible, determina que su realización en todos los pacientes no sea siempre posible. Mamografía y ecografía son métodos diagnósticos por imágenes siempre disponibles y más baratos, pero no se han identificados los parámetros para establecer un pronóstico adverso en el ION-SOLCA Objetivos: Establecer la capacidad predictiva de mal pronóstico de la mamografía y la ecografía en pacientes con CMTN. Metodología: Se realizó un estudio retrospectivo analítico en esta institución de Guayaquil, Ecuador que incluyó de manera no aleatoria por conveniencia 112 casos consecutivos con diagnóstico confirmado de CMTN, que cumplieron con los criterios de selección. Para el análisis estadístico se estimó, sensibilidad, especificidad, valores predictivos positivos y negativos, coeficiente de correlación positivos Resultado: Entre las pacientes con CMTN se reportó estables 50%, fallecidas 42%, recidivas 8% y metástasis 1.8%. El mal pronóstico se informó por ecografía en el 50.9%, por mamografía en el 59.8% y la evolución reportó un 50%. La valoración de la ecografía reportó RG %, S 73%, E 67%, VPP 72%, VPN 69%, CPP 2.24 y CPN 0.39. La valoración de la mamografía reportó un RG 69%, S 79%, E 59%, VPP, 66%, VPN 73%, CPP 1.91 y CPN 0.36. Conclusiones: En el ION-SOLCA La capacidad predictiva de mal pronóstico en CMTN de la ecografía y la mamografía es similar.

Pronóstico Background: Establishing pretreatment prognosis in triple-negative breast carcinoma (TNBC) allows directing therapeutic management. Magnetic reasoonance imaging fulfills this purpose. However, the cost associated with its development and not always being available, determines that its performance in all patients is not always possible. Mammography and ultrasound are diagnostic imaging methods that are always available and cheaper, but the parameters for establishing and adverse prognosis in ION-SOLCA have not been identified. Objectives: To establish the predictive capacity of poor prognosis of mammography and ultrasound in patients with TNBC. Methodology: A retrospective analytical study was carried out in this institution in Guayaquil, Ecuador, which included 112 consecutive cases with a confirmed diagnosis of TNBC that met the selection criteria. For the statical analysis, sensitivity, specificity, positive and negative predictive values, positive correlation coefficient, positive and negative predictive values, positive and negative likelihood ratio. Result: Among patients with TNBC, 50% were reported stable, 42% died, recurrences 8% and metastases 1.8%. Poor prognosis was reported by ultrasound in 50,9%, by mammography in 59.8% and evolution reported 50%. Ultrasound assessment reported GR%, S73%, E67%, PPV 66%, NPV 73 %, LR+ 1.91 and LR- 0.36. Conclusions: In ION-SOLCA, the predictive ability of ultrasound and mammography for poor prognosis in TNBC is similar.
