Utilidad de la ecocardiografía Y NT-PROBNP como predictores de insuficiencia cardiaca de novo en síndrome coronario agudo

La insuficiencia cardíaca aguda está asociada a síndrome coronario agudo. Ante la falta de investigaciones que validen la efectividad de las intervenciones diagnósticas en complicaciones asociadas al infarto tipo I se desarrolló la presente en el Hospital Dr. Abel Gilbert Pontón de Enero a Diciembre del 2019 analizando pacientes con insuficiencia cardíaca pos síndrome coronario agudo. El tipo de investigación fue: Analítico - Correlacional, retrospectivo, no experimental, de corte transversal, donde se analizó la utilidad de la Ecocardiografía y NT-PROBNP, para la validación estadística se usó prueba de Chi Cuadrado determinándose adecuada utilidad para ambas. Los hallazgos demostraron: Edad 64 años, Disnea 60 %, Ortopnea 52.3 %, Fatiga moderada 80 % y dolor anginoso 24.6 %. El 100 % presentaron flujo alterado en la cinecoronografía, el 78.5 % tuvo trastornos de la motilidad con un 60 % de tipo segmentario. La disfunción diastólica fue Tipo I en un 56.9 % y Tipo II con 30.8 %. El valor crítico para NT PROBNP fue superior a 125 pg/ml, en la Ecocardiografia se observó que un 81.54 % presentó sintomatología vs un 18.46 % que no lo hizo. El 12.3 % presentó una clase funcional alterada al alta vs un 87.7 % que estuvo normal, la FEVI al control ecocardiográfico a las 24 horas fue alterado en el 49.2 %. NT PROB NP > 125 pg/ml tuvo una sensibilidad del 71.93 % y especificidad del 87.50 %; mientras que la ecocardiografía tiene una sensibilidad del 86.44 % y especificidad del 66.67 %. Given the lack of research that validates the effectiveness of diagnostic interventions for heart failure as complications associated with type I infarction, the present investigation was carried out at the Dr. Abel Gilbert Pontón Hospital from January 1, 2019 to January 31 December 2019, made up of patients with a diagnosis of heart failure after acute coronary syndrome, excluding those who do not have a complete clinical history or those in whom the tests to be evaluated have not been followed, being an investigation of the type: Analytical - Correlational, retrospective, non-experimental, cross-sectional, where the usefulness of Echocardiography and NT-PROBNP is analyzed in patients treated at the Dr. Abel Gilbert Pontón Specialty Hospital of the City of Guayaquil. For this, a type statistical test was used. Chi Square determining the adequate utility of these tests. Among the findings of interest were: Mean age 64 years, associated symptoms: Dyspnea 60%, Orthopnea 52.3%, Moderate fatigue 80% and anginal pain 24.6%. 100% of patients presented altered flow in the cinecoronography, 78.5% had motility disorders of which 60% are segmental. Regarding diastolic dysfunction, it is 56.9% Type I and 30.8% Type II. The critical value for NT PROBNP was higher than 125 pg / ml, in relation to the echocardiographic correlation it was observed that 81.54% presented symptoms vs 18.46% who did not. 12.3% presented an altered functional class at discharge vs 87.7% who were normal, LVEF at 24-hour echocardiographic control was altered in 49.2%. NT PROB NP> 125 pg / ml had a sensitivity of 71.93% and specificity of 87.50%; while echocardiography has a sensitivity of 86.44% and a specificity of 66.67%
