Evaluación de la microfiltración apical con técnicas de obturación con gutapercha alfa con beta

Este trabajo está encaminado a incentivar la adopción de la técnica de obturación que presente menor nivel de microfiltración, por estudiantes de pregrado, postgrado y especialistas en endodoncia, mitigando así el fracaso endodóntico y por supuesto logrando resultados mucho más alentadores y óptimos a futuro. OBJETIVO: Determinar los niveles de microfiltración apical que tienen lugar en las distintas técnicas de obturación con gutapercha alfa con beta. METODOLOGÍA: El enfoque de esta investigación fue de tipo bibliográfico, usando métodos analítico-sintético, deductivo, y descriptivo. Se hizo la revisión bibliográfica de artículos y fuentes científicas, libros y repositorios, recolectando y analizando resultados relevantes para este trabajo. RESULTADOS: El reto de la terapia endodóntica consiste en obturar tridimensionalmente los conductos radiculares junto a sus complejidades de manera eficaz, manteniendo un control apical preciso del material de obturación. Estudios han demostrado que la gutapercha es bien tolerada por los tejidos, pero también existen muchos estudios que demuestran que todos los materiales y técnicas utilizadas para este fin permiten microfiltraciones. CONCLUSIONES: Se logró determinar que, la microfiltración apical sigue siendo la principal causa de fracaso endodóntico; ningún material y ninguna de todas las técnicas de obturación puestas en consideración presentan cero niveles de microfiltración apical, por lo tanto, ninguna de estas técnicas garantizan resultados totalmente satisfactorios. Las técnicas de obturación endodóntica que presentan niveles de microfiltración apical (de manera creciente), son las termoplastificadas, específicamente, Condensación Híbrida o Mixta, Obtura II y Condensación vertical, seguidas por las de Condensación Lateral, Termomecánica, Ultrasonido, y la de Cono Único This work is focused on encouraging the adoption of the obturation technique with the lowest level of microleakage by undergraduate and postgraduate students and endodontic specialists, thus mitigating endodontic failure and, of course, achieving much more encouraging and optimal results in the future. OBJECTIVE: To determine the levels of apical microleakage that occur in the different techniques of obturation with alpha with beta gutta-percha. METHODOLOGY: The approach of this research was bibliographic, using analytical-synthetic, deductive and descriptive methods. A bibliographic review of articles and scientific sources, books, and repositories was made, collecting and analyzing results relevant to this work. RESULTS: The challenge of endodontic therapy consists of three-dimensionally obturating root canals and their complexities effectively while maintaining precise apical control of the obturation material. Studies have shown that gutta-percha is well tolerated by tissues, but many studies show that all materials and techniques used for this purpose allow microleakage. CONCLUSIONS: It was determined that apical microleakage continues to be the main cause of endodontic failure; no material and none of the obturation techniques considered present zero levels of apical microleakage, therefore, none of these techniques guarantee satisfactory results. The endodontic obturation techniques that present levels of apical microleakage (in an increasing manner) are the thermoplastic ones, specifically, Hybrid or Mixed Condensation, Obtura II and Vertical Condensation, followed by Lateral Condensation, Thermomechanical, Ultrasound, and Single Cone.
