Efectividad de la amiodarona en pacientes ambulatorios con diagnóstico de fibrilación auricular

La fibrilación auricular (FA) es la arritmia cardíaca de mayor prevalencia entre todas las demás arritmias, y se caracteriza por ser una problemática de salud ya que disminuye la calidad de vida del paciente quitándole funcionalidad para sus actividades diarias debido a las secuelas producidas por los eventos cerebrovasculares (ECV), las recurrencias de los episodios de FA que terminan elevando la morbilidad y mortalidad de estos individuos. Objetivo: Determinar la efectividad de la amiodarona en pacientes ambulatorios con diagnóstico de fibrilación auricular. Metodología: Observacional, descriptivo, longitudinal retrospectivo. Lugar: Hospital de Especialidades “Teodoro Maldonado Carbo”. Periodo: 2017 – 2018. Procedimiento: Recolección de datos de las historias clínicas a partir el programa digital integral del Instituto Ecuatoriano de Seguridad Social (IESS), AS400. Resultados: De los 485 pacientes con FA, 67 fueron los que cumplieron los criterios de inclusión. De estos 67 pacientes, el 10% fue hospitalizado en el año 2017 y el 12% en el año 2018, esto implicó en total 8 ingresos se dieran durante el 2017 y 11 durante el 2018; la mortalidad cardiovascular fue el 4%, mientras que solo el 4% del total desarrollo ECV durante el tiempo de estudio. Conclusiones: La efectividad de la amiodarona se vio reflejada en que no aumenta la mortalidad cardiovascular ni los ECV en los pacientes que recibieron este medicamento, aunque el porcentaje de pacientes ingresados por año si son valores ligeramente preocupantes, tampoco se puede desmerecer su efectividad ya que los ingresos al año fueron bajos. Atrial fibrillation (AF) is the most prevalent cardiac arrhythmia among all other arrhythmias, and is characterized by being a health problem since it reduces the quality of life of the patient by taking away functionality for their daily activities due to the sequelae produced by the cerebrovascular events (CVE), recurrences of AF episodes that end up increasing the morbidity and mortality of these individuals. Objective: To determine the effectiveness of amiodarone in outpatients with a diagnosis of atrial fibrillation. Methodology: Observational, descriptive, retrospective longitudinal. Place: Hospital of Specialties "Teodoro Maldonado Carbo". Period: 2017 - 2018. Procedure: Collection of data from medical records from the comprehensive digital program of the Ecuadorian Institute of Social Security (IESS), AS400. Results: Of the 485 patients with AF, 67 were those who met the inclusion criteria. Of these 67 patients, 10% were hospitalized in 2017 and 12% in 2018, this implied a total of 8 admissions were given during 2017 and 11 during 2018; cardiovascular mortality was 4%, while only 4% of the total CVE development during the study time. Conclusions: The effectiveness of amiodarone was reflected in that it does not increase cardiovascular mortality or CVE in patients who received this drug, although the percentage of patients admitted per year is slightly worrisome values, nor can its effectiveness be diminished since earnings per year were low.
