Osteonecrosis Maxilar inducida por Bisfosfonatos: Epidemiología y caracterización clínica.

Introducción: Los bisfosfonatos son antirresortivos utilizados en el tratamiento de
padecimientos ligados con degradación de la matriz ósea: osteoporosis, enfermedad de Paget y
enfermedades oncológicas que afectan al tejido óseo. Estos inhiben y producen apoptosis de los
osteoclastos, deteniendo la reabsorción ósea. La osteonecrosis maxilar es un efecto adverso
característico del fármaco, el odontólogo cumple un papel fundamental en el diagnóstico
oportuno de la lesión y, mediante una adecuada atención permite minimizar los riesgos de
aparición. Objetivo: Identificar los factores epidemiológicos y clínicos observados en la
osteonecrosis maxilar inducida por bisfosfonatos. Metodología: Cualicuantitativa, no
experimental, documental, descriptiva, explicativa, se basó en la revisión sistemática en motores
de búsqueda especializados, acerca de las características epidemiológicas y clínicas de la
osteonecrosis maxilar inducida por bisfosfonatos. Resultados: Más frecuente en bisfosfonatos
intravenosos. Registró predilección en pacientes con cáncer de mama, seguido de mieloma
múltiple y osteoporosis. Hubo predilección en mujeres y en la edad de sesenta años. Los factores de riesgo locales registrados: extracciones dentales, traumas protésicos, implantes, endodoncias,
enfermedad periodontal y pobre higiene dental. Principalmente localizado en mandíbula, los
signos y síntomas fueron: exposición de hueso, dolor, infección, comunicación oroantral, fístula,
fractura, trismus y parestesia. El estadio de evolución más frecuente fue 2. Conclusiones: La
ONM tiene baja prevalencia, el grupo vulnerable son mujeres de sesenta años, con prescripción
intravenosa por tratamiento oncológico, suele aparecer luego de procedimientos dentales
invasivos, localizada frecuente en la mandíbula, el signo característico es la exposición de hueso,
pero, otros signos y síntomas permiten su diagnóstico oportuno. Introduction: Bisphosphonates are antiresorptive agents used in the treatment of

osteoclasts, stopping bone resorption. Maxillary osteonecrosis is a characteristic adverse effect of
the drug, the dentist plays a fundamental role in the timely diagnosis of the injury, and, through
adequate care, it allows minimizing the risks of its appearance. Objective: Identify the
epidemiological and clinical factors observed in bisphosphonate-induced osteonecrosis of the
jaw. Methodology: Qualitative-quantitative, non-experimental, documentary, descriptive,
explanatory, it was based on a systematic review in specialized search engines, about the
epidemiological and clinical characteristics of bisphosphonate-induced osteonecrosis of the jaw.
Results: More frequent in intravenous bisphosphonates. It registered a predilection in patients
with breast cancer, followed by multiple myeloma and osteoporosis. There was a predilection in
women and in age of sixty years. Registered local risk factors: dental extractions, prosthetic trauma, implants, root canals, periodontal disease, and poor dental hygiene. Mainly located in the
mandible, the signs and symptoms were bone exposure, pain, infection, oroantral
communication, fistula, fracture, trismus, and paresthesia. The most frequent stage of evolution
was 2. Conclusions: ONJ has a low prevalence, the vulnerable group is women in their sixties,
with an intravenous prescription for cancer treatment, it usually appears after invasive dental
procedures, frequently located in the jaw, the characteristic sign is bone exposure, but other signs and symptoms allow their timely diagnosis.
