Complicaciones clínicas frecuentes en pacientes con mordeduras de serpiente del Hospital de Infectología José Daniel Rodríguez Maridueña en el año 2017-2019 y respuesta al suero antiofídico

Las complicaciones por accidentes ofídicos en nuestro país sobre todo a nivel de Costa y Amazonía son frecuentes debido a la presencia de ciertas especies venenosas. Existen alrededor de 230 especies, dentro de las cuales son de interés toxicológico: Elapidae y Viperidae. El objetivo de la presente investigación es determinar las complicaciones clínicas frecuentes debido a las mordeduras de serpientes en el Hospital de Infectología de la Ciudad de Guayaquil y la respuesta que tuvieron al tratamiento con suero antiofídico. El presente estudio es de tipo transversal, descriptivo y analítico basado en pacientes que se complicaron debido al accidente ofídico haciendo un total de 44 pacientes. Los resultado concluyeron que las más frecuentes complicaciones clínicas fueron alteraciones en los tiempos de coagulación 23%, trombocitopenia 19%, infección local 9%, alteraciones neurológicas 6%, en un 5% insuficiencia renal aguda y síndrome compartimental, 4% tuvieron gingivorragias, ACV hemorrágico, necrosis, perdida de tejido y reacción alérgica al suero antiofídico, 2% cursaron con shock anafiláctico y el 1% presentó otras complicaciones como septicemia, neumonía, derrame pleural, edema agudo de pulmón, rabdomiólosis, alteraciones dermatológicas, incluso la muerte. Así se deduce que pese a que no todos presentan complicaciones graves, cuando se presentan es importante manejar adecuadamente al paciente, estabilizarlo y valorar cuando necesita de suero antiofídico para neutralizar el veneno. Por otro lado, hay que concientizar a la población sobre el evitar malas prácticas que solo empeoren el cuadro clínico del paciente. Complications from ophidian accidents in our country, especially at the level of the Coast and Amazon, are frequent due to the presence of certain poisonous species. There are about 230 species, among which are of toxicological interest: Elapidae and Viperidae. The objective of this investigation is to determine the frequent clinical complications due to snake bites in the Hospital of Infectology de la Ciudad de Guayaquil and the response they had to treatment with antivenom. The present study is cross-sectional, descriptive and analytical based on patients who became complicated due to the ophidian accident, making a total of 44 patients. The results concluded that the most frequent clinical complications were alterations in clotting times 23%, thrombocytopenia 19%, local infection 9%, neurological alterations 6%, in 5% acute renal failure and compartment syndrome, 4% had gingivorrhagia, stroke hemorrhagic, necrosis, loss of tissue and allergic reaction to antivenom, 2% had anaphylactic shock and 1% had other complications such as septicemia, pneumonia, pleural effusion, acute lung edema, rhabdomyllosis, dermatological alterations, even death. Thus, it follows that although not all present serious complications, when they do occur it is important to properly manage the patient, stabilize him and assess when he needs antivenom to neutralize the venom. On the other hand, the population must be made aware of avoiding bad practices that only worsen the patient's clinical picture
