Tratamiento de la fractura mandibular mediante el Sistema Michelet-Champy

Las fracturas mandibulares se producen debido a su ubicación anatómica, a que se encuentran en un área vulnerable, o a causa de que está expuesto a traumatismos. En la atención multidisciplinaria de urgencia, en los Servicios Hospitalarios, se debe garantizar la permeabilidad de las vías respiratorias, controlar el sangrado y estabilizar los segmentos de las fracturas. Posteriormente en el tratamiento definitivo, en el caso de las fracturas de ángulo mandibular y en la sínfisis se recomienda la técnica de Michelet-Champy, ya que se puede realizar bajo anestesia local la colocación de fijación rígida intraoral, considerando esta atención ambulatoria. Las complicaciones de los traumatismos dependen de la intensidad del agente vulnerante, del factor dentario, de la línea de fractura que determina mayor desplazamiento de los segmentos óseos y de la oportuna atención mediante la reducción, fijación y mantenimiento de la fractura. El Objetivo de este trabajo es determinar las fracturas que pueden recibir el tratamiento mediante el sistema Michelet-Champy. Metodología: Enfoque cualitativo, exploratorio, estudio bibliográfico de revistas electrónicas como Scielo, Elsevier, Medigraphic. Método inductivo-deductivo, analítico-sintético. Los Resultados muestran la eficacia del tratamiento mediante la técnica de Michelet-Champy en las fracturas de ángulo mandibular y parasinfisarias. Se pueden observar pocos casos con infecciones en el foco de la fractura debido a la exposición de las miniplacas. Conclusión la técnica de Michelet-Champy ofrece ventajas como el abordaje intraoral, disminuyendo el tiempo de recuperación, además de que puede ser realizado bajo anestesia local, de manejo ambulatorio, lo que disminuye considerablemente los costos en la atención de los traumatismos mandibulares. Later in the definitive treatment, in the case of mandibular angle fractures and in the symphysis, the Michelet-Champy technique is recommended, since intraoral rigid fixation placement can be performed under local anesthesia, considering this outpatient care. The complications of the trauma depend on the intensity of the vulnerable agent, the dental factor, the fracture line that determines the greatest displacement of the bone segments, and the timely attention through reduction, fixation and maintenance of the fracture. The objective of this work is to determine the fractures that can be treated using the Michelet-Champy system. Methodology: Qualitative, exploratory approach, bibliographic study of electronic journals such as Scielo, Elsevier, Medigraphic. Inductive-deductive, analytical-synthetic method. The results show the efficacy of the treatment using the Michelet-Champy technique in mandibular angle and parasymphyseal fractures. Few cases with infections at the fracture site due to mini-plate exposure can be seen. Conclusion The Michelet-Champy technique offers advantages such as the intraoral approach, reducing the recovery time, in addition to the fact that it can be performed under local anesthesia, with outpatient management, which considerably reduces costs in the care of jaw trauma.
