Antecedentes: El amamantamiento humano se asocia a innumerables beneficios para el neonato. Sin embargo, esta actividad puede ser interferida por un sinnúmero de situaciones. Como respuesta el Ministerio de Salud Pública (MSP) del Ecuador impulsa el apego precoz y la lactancia pero se desconoce la asociación del tipo de parto y la conducta de amamantamiento en éste ámbito. Objetivo: Conocer la repercusión que tiene la vía de finalización del parto sobre la conducta de amamantamiento humano entre mujeres cuyo parto fue atendido en el hospital Sagrado Corazón de Jesús del Ministerio de Salud Pública del Ecuador. Metodología: se desarrolló un estudio de cohortes en el que se hizo seguimiento por 30 días de las conductas de amamantamiento de 37 mujeres con parto, 11 partos vaginales PV y 26 cesáreas (PV) atendidos en la institución. Análisis estadístico: Para la comparación de las variables se emplearon Las pruebas student-Fisher y chi-cudrado y Medidas de impacto (ARA, RR) considerándose significativos valores de p < 0.05. Resultados: En las primeras 24 horas las horas de inicio de lactancia se retrasaron más en el PC que en el PV (p < 0.05), y el número de amamantamientos efectivos fue más frecuente en el PV (p < 0.05). En los 30 días posparto los días transcurridos hasta el inicio de lactancia efectiva fueron mayores en el PC < 0.05). El uso de fórmula también fue mayor en el grupo de PC (p< 0.05). El número de amamantamiento promedio día fue menor en el PC (p< 0.001). El nivel de dolor fue mayor entre pacientes con PC y PV así como el tiempo de inicio de lactancia (p<0.001) y un mayor uso de fórmula (p < 0.01). La edad no influyó. Conclusiones: La conducta de amamantamiento se ve influenciada por el tipo parto. Posiblemente el factor más importante es el dolor ya que el PC se relaciona con un mayor dolor postquirúrgico. Statistical analysis: To compare the variables Student-Fisher and chi tests were used, and ARI, RR for impact measures. A p value <0.05 were considered significant. Results: In the first 24 hours, the lactation times in the CD were more delayed than in the VD (p <0.05), and the number of effective breastfeeding was more frequent in the VD (p <0.05). Within 30 days postpartum, the days until the start of effective breastfeeding were higher in the PC <0.05). The use of formula was also higher in the CD group (p <0.05). The average number of daily breastfeeding was lower in the CD (p <0.001). The level of pain was higher among patients with VD and CD and start time of breastfeeding (p <0.001) and greater use of formula (p <0.01). Age did not influence. Conclusions: Breastfeeding behavior is influenced by delivery type. Possibly the most important factor is the pain because the CD is associated with increased postoperative pain.
