Hemodiálisis urgente en el Hospital del IESS de Portoviejo

Antecedentes: La hemodiálisis es la terapéutica más frecuente de la Enfermedad Renal
Crónica (ERC). Existen muchos pacientes con indicación de urgencia dialítica sin acceso vascular,
por una atención nefrológica inoportuna, originando situaciones adversas que comprometen la
evolución del paciente. Objetivo: Estimar la atención nefrológica en pacientes con ERC, con
indicación de hemodiálisis urgente en el hospital del IESS de Portoviejo, en el 2016. Objetivo
Específico: Identificar pacientes con ERC, con necesidad urgente de hemodiálisis por primera vez.
Metodología: cuantitativa, de método descriptivo, diseño no experimental, de corte
trasversal, y de carácter retrospectivo, almacenando la información de historias clínicas en un
recolector de datos (N: 110). Resultados: Se observó que la hemodiálisis urgente alcanzó un 9% en
pacientes con Fistula Arteriovenosa (FAV), y 91% en pacientes sin FAV. La edad más frecuente
fue entre 30 – 59 años que correspondió al 58%. En las patologías acompañantes la diabetes mellitus
alcanzó un 46% y la Hipertensión Arterial 27%. La hospitalización <5 días en pacientes con FAV
representó el 80%, y en pacientes sin FAV comprendió el 28%. El inicio de Hemodiálisis <12 hs
en pacientes con FAV abarcó el 70%, y sin FAV el 37%. En complicaciones de pacientes sin FAV
el fallecimiento < 5 días tuvo 5,5%, y signos de infección el 57%. Conclusiones: La atención
nefrológica inoportuna conlleva la creación de acceso vascular urgente, prolonga el tiempo de
hospitalización, retrasa el inicio del tratamiento, y aumenta el número de complicaciones en estos
pacientes. Background: Hemodialysis is the most common therapy for chronic renal disease (CKD). There
are many patients with indication of dialysis urgency without vascular access, due to an untimely
nephrological care, causing adverse situations that compromise the evolution of the patient.
Objective: To estimate nephrological care in patients with CKD, with indication of urgent
hemodialysis at the IESS hospital in Portoviejo, in 2016. Methodology: quantitative, descriptive
method, non-experimental design, cross-sectional, retrospective, storing the clinical history
information in a data collector (N: 110). Results: It was observed that urgent hemodialysis reached
9% in patients with AVF, and 91% in patients without AVF. The most frequent age was between
30 - 59 years corresponding to 58%. In the accompanying pathologies diabetes mellitus reached
46% and arterial hypertension 27%. The hospitalization <5 days in patients with AVF represented
80%, and in patients without AVF comprised 28%. The onset of Hemodialysis <12 hours in patients
with AVF comprised 70%, and without AVF 37%. In complications of patients without AVF, the
death <5 days had 5.5%, and signs of infection 57%. Conclusions: Inappropriate nephrological care
leads to the creation of urgent vascular access, prolong hospitalization time, delay the start of
treatment, and increase the number of complications in these patients.
