Patógenos encontrados en cultivos de heridas quirúrgicas que no responden al tratamiento

Resumen
Las infecciones de heridas quirúrgicas son las complicaciones mas frecuentes que se
presentan en los pacientes que han pasado por algún tipo de procedimiento quirúrgico,
estas representan un gran problema de salud pública ya que se traducen en largas estancias
hospitalarias, el uso de recursos económicos, materiales y humanos; en ocasiones
alcanzando incluso hasta la muerte. El presente trabajo de investigación tiene como
objetivo Identificar el agente patógeno más frecuente aislado en cultivos de heridas
quirúrgicas infectadas que no responden al tratamiento en el periodo 2018 hasta 2019 en el
Hospital General Guasmo Sur. La metodología que se utilizo es de enfoque cuantitativo,
diseño no experimental, de corte longitudinal retrospectivo y tipo analítico, para la
realización de este proyecto se recolectaron datos de 441 pacientes que fueron tratados de
manera quirúrgica en el Hospital General Guasmo Sur en el periodo comprendido entre abril
de 2018 a enero de 2019, los cuales presentaron infección de herida quirúrgica según los
criterios establecidos, se les realizo cultivo de tejido o de secreción que drenaba de las
mismas obteniendo como resultado que la bacteria gramnegativa Escherichia coli es el
agente patógeno más comúnmente aislado en este tipo de complicaciones, a diferencia de
los resultados mundiales estipulados por la organización mundial de la Salud que indica que
el patógeno más frecuente es la bacteria grampositiva Estafilococcus aureus. La Escherichia
coli encontrada en estos cultivos de secreciones y tejidos resulto tener mayor resistencia
antibiótica para los betalactamicos y ciprofloxacino; y menor resistencia para colistin,
amikacina y carbapenemicos. Infections of surgical wounds are the main complication that occurs in patients who have
undergone some kind of surgical procedure. These represent a great public health problem
since they result in long hospital stays, the use of economic resources, materials and
humans; sometimes reaching even to death. The objective of this research work is to
identify the most frequent pathogen isolated in cultures of infected surgical wounds that do
not respond to treatment in the 2018 to 2019 period in the Guasmo Sur General Hospital.
The methodology used is a quantitative approach, non-experimental design, retrospective
longitudinal cut and analytical type, for the realization of this project data were collected
from 441 patients who were treated surgically in the general hospital south Guasmo in the
period included between April 2018 and January 2019, which presented infection of the
surgical wound according to the established criteria, they underwent tissue culture or
secretion that drained from them, obtaining as a result that the Gram-negative bacterium
Escherichia Coli is the most common pathogen isolated in this type of complications, unlike
the world results stipulated by the World Health Organization that indicates that the most
frequent pathogen is the Gram-positive bacteria Staphylococcus aureus. The Escherichia Coli
found in these cultures of secretions and tissue resulted to have greater antibiotic
resistance for the betalactamics and Ciprofloxacin; and lower resistance for Colistin,
Amikacin and Carbapenemics.
