La extracción de terceros molares suele ser un procedimiento complejo debido a que estos muestran más variaciones anatómicas radiculares e interrupciones en su erupción en comparación a otras piezas provocando impactaciones indeseadas, tomando en cuenta que su lugar de posición es un acortado espacio en la parte más posterior por detrás del segundo molar. Este presenta diversas formas radiculares como raíces divergentes, convergentes, fusionadas, etc., que podrían crear un impedimento para la avulsión del mismo. Al querer extraer un tercer molar inferior es necesario tener presente las estructuras anatómicas que rodean dicha zona, las cuales son de vital importancia. Objetivo: Determinar la eficacia de la técnica quirúrgica con Osteotomía aplicada en la exodoncia de tercer molar inferior con anomalía radicular. Método: Cualitativo-No experimental, Descriptivo-Transversal. Se realizó la exodoncia de un tercer molar inferior mesioangular según Winter y Pell y Gregory posición 3A, con dilaceración radicular donde se aplicó la técnica de la Osteotomía para eliminar la impactacion y luego separar el diente del hueso permitiendo vencer el anclaje radicular provocado por la dilaceración, utilizando fresas quirúrgicas de carburo de tungsteno e instrumentos rotatorios de baja velocidad con una irrigación constante todo en un menor tiempo de intervención y sin complicaciones. Conclusiones: Para realizar la exodoncia de tercer molar con anomalía radicular es importante elaborar una Historia clínica completa con diagnóstico clínico-radiográfico certero y una buena técnica quirúrgica acompañada de un buen manejo del instrumental que dará como resultado una intervención impecable. The extraction of third molars is usually a complex procedure because they show more anatomical variations and radicular interruptions in their eruption compared to other pieces causing unwanted impacts, taking into account that their place of position is a shortened space in the posterior part behind the second molar. This presents diverse root forms as divergent roots, convergent, fused, etc. that could create an impediment to the avulsion of it. When wanting to extract a third lower molar, it is necessary to bear in mind the anatomical structures that surround this area, which are of vital importance. The objective of this study is to determine the efficacy of the surgical technique with Osteotomy applied in the extraction of inferior third molar with root anomaly. The research design is qualitative-non-experimental, descriptive-transversal. Extraction of a third lower molar mesioangular according to Winter and Pell and Gregory position 3A was performed, with root dilacerations where the technique of Osteotomy was applied to eliminate the impaction and then separate the tooth from the bone allowing to overcome the root anchorage caused by the dilacerations, using tungsten carbide surgical drills and low speed rotary instruments with constant irrigation all in a shorter intervention time and without complications. Over all, to perform the third molar extraction with radicular anomaly it is important to elaborate a complete clinical history with accurate clinical-radiographic diagnosis and a good surgical technique accompanied by a good handling of the instruments that will result in an impeccable intervention.
