Nutrición parenteral y sus complicaciones en el recién nacido de alto riesgo. Hospital Regional IESS Dr. Teodoro Maldonado Carbo 2009-2010

La incapacidad para alimentar a los niños, especialmente a los recién nacidos debido a diversas patologías, es un problema para el médico pediatra, ya que las indicaciones para proporcionar nutrientes por vía parenteral no están bien definidas y la eficacia del soporte nutricional en muchas circunstancias no está documentado, es por esto que se debe implantar un sistema de alimentación parenteral programado, fácil de aplicar que favorezca el crecimiento y desarrollo del paciente. Es importante conocer que la alimentación parenteral no es inocua y existen situaciones en que se presentan complicaciones las que pueden ser de tipo infeccioso, metabólico, entre otras. Este trabajo tuvo como objetivo determinar las características clínicas y las complicaciones en los recién nacidos de alto riesgo y proponer un protocolo de nutrición parenteral, en pacientes ingresados en la unidad de terapia intensiva neonatal del Hospital Regional IESS Dr Teodoro Maldonado Carbo de la ciudad de Guayaquil. La metodología del estudio fue de tipo exploratorio , descriptivo, correlacional , diseño no experimental longitudinal prospectivo . Entre los resultados obtenidos indican que el sexo no fue un factor determinante de complicaciones aunque se encontró un ligero predominio del sexo masculino (60%), la mayoría de los casos se trato de pretérminos y de bajo peso lo cual si tuvo relación directa en un 69% y de ellas la causa metabólica ocupo el primer lugar, seguido de las complicaciones infecciosas. La estancia hospitalaria promedio fue de 12 días lo cual estuvo relacionado directamente con la prematurez y el bajo peso al nacer. La mortalidad alcanzó 12%. Se pudo concluir que la alimentación parenteral total o parcial es sin duda un instrumento de enorme importancia para el crecimiento y desarrollo del recién nacido y que la técnica y la implantación de protocolos disminuirán las complicaciones en los pacientes que reciben alimentación parenteral total, lo que contribuye a bajar la morbimortalidad neonatal. The disability to feed the children, specially to the newborn children due to diverse pathologies, is a problem for the medical pediatrician, since the indications to provide nutrients for route parenteral are not definite well and the efficiency of the nutritional support in many circumstances is not documented, is for this that must implant a system of supply parenteral programmed, easily to apply that it favors the growth and development of the patient. It is important to know that the supply parenteral is not innocuous and there exist situations in which they present complications those that can be of infectious, metabolic type, between others. Our work had as aim determine the clinical characteristics and the complications in the newborn children of high risk and propose a protocol of nutrition parenteral, in patients joined the unit of intensive therapy neonatal of the Hospital of the social insurance Dr Teodoro Maldonado Carbo of the city of Guayaquil . The methodology of the study was market, longitudinal, descriptive not experimentally. Between the obtained results they were tabulated statistically and we can say that the sex was not a determinant factor of complications though one found a light predominance of the masculine sex (60 %), the majority of the cases I treat of pretérminos and of low weight which if it had direct relation with the presence of complications, the frequency of complications was 69 % and of them the metabolic reason I occupy the first place, followed by the infectious complications. The hospitable average stay of our patients was 12 days which was related directly to the prematurez and the low weight on having been born. The mortality was 12 %. It was possible to conclude that the supply parenteral total or partial is undoubtedly an instrument of enormous importance for the growth and development of the newborn child and that the technology and the implantation of protocols will diminish the complications in the patients who receive supply parenteral total, diminishing of this form the morbimortalidad neonatal
