Incidencia de displasias en pacientes en edad reproductiva en el Servicio de Colposcopía de la Consulta Externa Hospital Gineco-Obstétrico Enrique C. Sotomayor 2012-2014

Abstract

La displasia cervical es una patología con grave consecuencia a la paciente. Es una condición precancerosa en la que el crecimiento celular anormal se produce en el revestimiento de la superficie del cuello del útero, la apertura entre el útero y la vagina. Durante el período del presente estudio se atendieron en el servicio de colposcopia de la consulta externa del Hospital Enrique C. Sotomayor un total de 11.382 pacientes por año. De este universo, un 1.75 % (n=200) de las pacientes poseen algún tipo de displasia por una causa que es el VPH. Se incluyeron pacientes embarazadas y no embarazadas de los cuales de los 200 casos recopilados 153 resultaron ser embarazos y se les interrumpió el embarazo por vía abdominal por infección VPH y otras por antecedentes de papiloma virus no tratados. Las 47 pacientes restantes resultaron ser no embarazadas y que en este grupo tuvieron que realizársele conización (21 casos) histerectomías (18 casos) y por último 8 de las 47 pacientes abandonaron su tratamiento, Se encontró en las anamnesis revisadas que las pacientes atendidas en el servicio de colposcopia de la consulta externa presentaron entre 1-2 parejas sexuales que represento 91% (n=185) mientras que el 6.5% (n=13) tuvieron 3 parejas sexuales y un 0.5% (n=1) 4 parejas sexuales. Las embarazadas después de que se le realizó la cesárea fueron citadas a la consulta externa al servicio de colposcopia para que sigan un tratamiento adecuado y así poder prevenir un cancer cérvico uterino en un futuro no muy lejano. Se ha demostrado también que en la etapa adulta los tejidos cervicouterino son más susceptibles a la acción de los carcinógenos, y de hecho, si existe un agente infeccioso relacionado, el tiempo de exposición a este será mucho mayor. El riesgo de lesión intraepitelial cuando el primer coito se tiene a los 17 años o menos es 2,4 veces mayor que cuando este se tiene a los 21 años.
