Relación entre la Carga Blástica y la Enfermedad Mínima Residual en pacientes pediátricos con Leucemia Linfoblástica Aguda B precursora en la Sociedad de Lucha contra el Cáncer (SOLCA) de Guayaquil durante el período de 2012 al 2014.

La Leucemia Linfoblástica Aguda de células B precursoras (LLA - B), es la neoplasia maligna más común en la infancia, y consiste en la proliferación anormal de células precursoras de los linfocitos B, que son elementos sanguíneos, involucrados en la defensa del organismo frente a sustancias nocivas. Constituye un tipo de leucemia muy agresiva, que ocasiona el funcionamiento deficiente de la médula ósea, debido a la invasión de las células patológicas y, a la disminución de las células normales. Sin tratamiento es una enfermedad potencialmente mortal; sin embargo, en la actualidad, y gracias a los avances médicos, presenta tasas de remisión de hasta el 85% en la población infantil. Esta patología se divide en tres subgrupos: la LLA con fenotipo Pro – B, Pre – B, y B – Común; lamentablemente no es previsible, y hasta el momento se desconocen las causas de su padecimiento en la niñez.
El propósito del presente trabajo, fue determinar la relación entre la carga blástica inicial en contraste a la enfermedad mínima residual, y otras variables como el subtipo de patología, el sexo, y la edad. Se analizó un total de 140 pacientes pediátricos, diagnosticados con LLA – B por Citometría de flujo, ingresados entre el 2012 y el 2014; de los cuales el 67% fue LLA – B Común, el 32% LLA – Pre – B, y el 1% LLA – Pro – B; así también, el 65% de la población fue de sexo masculino y el 35% femenino. El 50% de los pacientes fue del grupo etario de 2 a 5 años, el 26% del grupo de 6 a 9 años, el 16% entre 10 a 13 años y el 8% entre 14 a 17 años. En relación a la EMR posterior a la inducción, de los 140 pacientes evaluados, el 14% (20)

obtuvieron EMR de 0.01% (EMR negativa), mientras que el 64% (90), valores de EMR entre 0.02 y el 1%.
De los 140 pacientes, el 35% (49) presentaron recaída medular durante el tiempo de muestreo, el 9% recayó durante la etapa de tratamiento de inducción, y el 12% durante la etapa de consolidación.
Por otra parte, de los pacientes que tuvieron EMR de 0.01%, el 30% presentó recaída en etapas posteriores; de los que tuvieron entre 0,02 - 0,09%, el 24% recayó; de aquellos con valores entre ≥0,1 - 1%, el 31% manifestó recidiva; de igual manera, de los que tuvieron EMR entre >1 - 10%, el 48% presentó recaída, y finalmente todos los pacientes con EMR de >10% recayeron. Lo cual indica, que mientras mayores sean los valores de la EMR en la post- inducción, el riesgo de recaer en la enfermedad aumenta.
Se demostró que existe diferencia significativa entre la carga blástica inicial y la EMR 1 posterior a la quimioterapia de inducción (p<0.0001), y ambas se relacionan de manera directa. Sin embargo, el pronóstico clínico del padecimiento considera varios parámetros; por tanto, de manera independiente, la carga blástica, no permite inferir en la evolución de los pacientes frente a la enfermedad. Asimismo se determinó que los pacientes del género masculino, los niños entre 2 a 5 años y los pacientes con fenotipo Pro – B y Pre – B presentan mayor riesgo de recaídas. Finalmente, se concluye que las variables analizadas, en este tipo de patología, no son las únicas que podrían estar afectando el pronóstico de los pacientes con respecto a la enfermedad. B - Lineage Acute lymphoblastic leukemia (B - ALL) cells, it is the most common malignancy in childhood, and involves abnormal proliferation of precursor cells of B lymphocytes, which are blood elements involved in defending the body against to harmful substances. Is a very aggressive type of leukemia caused by the malfunction of the bone marrow, due to the invasion of pathological cells and, normal cells decreased. Without treatment is a life-threatening disease. However, today, thanks to medical advances, has remission rates of up to 85% in children. This pathology is divided into three subgroups: ALL with phenotype Pro - B, Pre - B and B - Common; unfortunately it is not a predictable illness, and so far the causes of this condition in childhood are unknown.
The purpose of this study was to determine the relationship between the initial blast load in contrast to the minimal residual disease, and other variables such as the subtype of disease, gender, and age. A total of 140 pediatric patients diagnosed with B - ALL by flow cytometry, admitted between 2012 and 2014 was analyzed; of which 67% was LLA - B Common, 32% LLA - Pre - B, and 1 % LLA - Pro - B; well, 65% of the population was male and 35% female. 50% of patients were in the age group 2 to 5 years, 26% of the group of 6 to 9 years, 16% between 10 to 13 years and 8% between 14 to 17 years.
In relation to the Minimal Residual Disease (MRD) after induction, the 14% (20) had levels of MRD of 0.01% (EMR negative), while 64% (90), had MRD

levels between 0.02 and 1%, of 140 patients evaluated. Of the 140 patients, 35% (49) had marrow relapse during the sampling time, fell 9% during the induction phase of treatment, and 12% during the consolidation phase.
Moreover, patients who had EMR 0.01%, 30% relapse in later presented; of those with between 0.02 to 0.09%, it fell 24%; those with values between ≥0,1 - 1%, 31% said recurrence; likewise, for those who had EMR in> 1 - 10%, 48% had relapsed, and finally all patients with EMR> 10% relapsed. This indicates, that the greater values of EMR in the post induction, the risk of relapse in the disease increases.
It was shown that there is significant difference between the initial blast loading and EMR 1 after induction chemotherapy (p<0.0001), and both are related directly. However, the clinical prognosis of the disease considers several parameters; therefore, independently, blast loading, not infer in the evolution of patients against disease. It was also found that male patients, children between 2 to 5 years and patients with phenotype Pro - and Pre B - B are at increased risk of relapse. Finally, we conclude that the variables analyzed in this type of pathology, are not the only ones that could be affecting the prognosis of patients.
