Efectos del bypass gástrico y del cruce duodenal corto sobre las complicaciones microvasculares en pacientes con Diabetes Mellitus tipo 2

Evaluar los efectos del bypass gástrico(BGYR) y del cruce duodenal corto (CDC) sobre las complicaciones microvasculares en pacientes con Diabetes Mellitus tipo 2 (DM-2). Método: se incluyeron 25 pacientes: a 10 se les realizó BGYR y a 10 CDC, los 5 restantes constituyen el grupo control; se realizaron estudios: microalbuminuria en orina de 24 horas, electromiografía, eco doppler carotídeo y fondo de ojo pre operatorio y al año del post operatorio. Resultados: el índice de masa corporal (IMC) disminuyó en el BPGY un 17,7kg/m2; en el CDC 15,3kg/m2; y en el grupo control 3,4kg/m2.La microalbuminuria disminuyó 1,7mg/24 horas en el BPGY; 8,9mg/24 horas en el CDC y en el grupo control incrementó un 1,2mg/24 horas;estadísticamente no significativo.En cuanto a la electromiografía, eco doppler carotídeo y fondo de ojo no se evidenciaron cambios entre el pre y post operatorio estadísticamente significativos. Conclusiones: el BPGY y el CDC son efectivos para la pérdida de peso, y aunque no se evidenció una regresión ni un cambio estadísticamente significativo en los parámetros evaluados, sí se ve una disminución o falta de progresión de los mismos, resultado que podría estar relacionado con el tamaño de la muestra y el tiempo de seguimiento. To evaluate the effects of gastric bypass (GB) and short duodenal switch (SDS) on the microvascular complications in patients with type 2 diabetes (DM-2). Methods: we included 25 patients: on 10 of them we performed GB, 10 SDS and 5 were the control group; we did the following studies: 24 hours microalbuminury, electromyography, carotid doppler ultrasound and fundoscopy in the pre operatory and 1 year after surgery. Results: the body mass index (BMI) diminished in the GB 17,7kg/m2, in the SDS 15,3kg/m2 and in the control group 3,4kg/m2. Microalbuminury diminished 1,7mg/24h in the GB, 8,9mg/24h in the SDS and increased 1,2mg/24h in the control group; which was no statistically representative. Conclusion: the GB and SDS are effective methods for loss of weight, and, even though we didn´t find regression or statistically representative changes with the studied parameters, we did find a little decrease or a lack of progression in themselves; this results could be explained by the small sample and the short following time.
