Resumen
El presente estudio de investigación tiene como objetivo determinar el efecto de un programa de seguimiento farmacoterapéutico sobre la adherencia al tratamiento en pacientes con hipertensión desde la Farmacia Dia y Noche. Chimbote, mayo –julio 2017. La muestra de 12 pacientes fue monitoreada a través del seguimiento farmacoterapéutico basado en una modificación de método DADER de cinco fases: captación, levantamiento de información, evaluación de la información, intervención, seguimiento y medición de resultados. El seguimiento farmacoterapéutico se dirigió a través del sistema de reunión individual, esta se llevó a cabo en la farmacia y en las casas de los pacientes. En cada fase se recogió información en herramientas e instrumentos específicos: Fichas fármaco terapéuticas, Escala para la adherencia (MAS) de Morisky –Green –Levine, Test de Macnemar para la significancia de la intervención. Resultados: se identificaron 39 PRMs y se solucionaron 39 iguales a (100 %). La adherencia comparada antes–después de la intervención indico un efecto poco significativo P=0.250. Por lo tanto, se concluye que el efecto de la intervención farmacéutica no fue eficaz en mejorar la adherencia al tratamiento en los pacientes hipertensos.
