Valor del recuento plaquetario y del diámetro mayor del bazo para el diagnóstico de várices esofágicas en adultos con cirrosis

OBJETIVO: Determinar el valor del recuento plaquetario y del diámetro mayor de bazo para el diagnóstico de várices esofágicas en adultos con cirrosis.
MATERIAL Y MÉTODO: Estudio de prueba diagnóstica. Se incluyeron 196 pacientes con diagnóstico de cirrosis hepática, con exámenes bioquímicos completos, valoración ecográfica del tamaño esplénico y endoscopía digestiva alta como patrón de oro. Se realizó análisis uni y multivariado de los parámetros paraclínicos como predictores de várices esofágicas.
RESULTADOS: La edad promedio fue de 62.7 ±12.9 años. El 54.1% de casos eran varones; el 51.5% de pacientes presentaron várices esofágicas. El recuento plaquetario y diámetro mayor de bazo presentaron áreas bajo la curva de 0.78 y 0.82 respectivamente para diagnosticar várices. Con puntos de corte óptimos, ≤150 000/μL (Sensibilidad 84.2% y especificidad 64.2%) para el primero y >140 mm (Sensibilidad 70.3% y especificidad 81.1%) para el segundo. Un modelo regresión logística incluyendo ambos parámetros tuvo un área bajo la curva ROC de 0.847, con sensibilidad y especificidad respectivas de 94% y 62.1%.
CONCLUSIONES: Los valores diagnósticos de várices esofágicas en adultos cirróticos del recuento plaquetario y del diámetro mayor de bazo son moderados; mejoran si se considera a ambos en un modelo, cuyo uso se recomienda
