Relación entre la hiperglicemia al ingreso hospitalario y la mortalidad en pacientes no diabéticos con infarto agudo de miocardio

Determinar si existe relación entre la hiperglicemia al ingreso hospitalario y la mortalidad en pacientes no diabéticos con infarto agudo de miocardio. Materiales y métodos: Estudio analítico, transversal, de cohorte histórica. Se revisaron historias clínicas de 150 pacientes adultos con diagnóstico de infarto agudo de miocardio, atendidos en el servicio de Emergencia del HRDT, donde constara la medición de sus niveles de glicemia al ingreso hospitalario. De acuerdo a los valores obtenidos se les incluyó en cualquiera de estos grupos: Niveles de glicemia ≥ 140 mg/dl y < 140 mg/dl. Se observó la evolución en todos los pacientes hasta un máximo de 30 días, para consignar su fallecimiento. Se utilizó la prueba no paramétrica de independencia de criterios Chi-cuadrado (X2) con un nivel de significancia del 5% (p < 0.05) y se determinó el Riesgo Relativo (RR) con un intervalo de confianza del 95 %. Resultados: La edad promedio de pacientes no diabéticos con infarto agudo de miocardio fue de 66 años y el sexo predominante fue masculino con un 84%. De los pacientes no diabéticos con infarto agudo de miocardio, un 23% de pacientes presentaron hiperglicemia al ingreso hospitalario. De los pacientes no diabéticos con infarto agudo de miocardio, un 20 % de pacientes murieron. Se obtuvo una Prueba de Chi cuadrado de 70,32 y un riesgo relativo de mortalidad por infarto agudo de miocardio en los pacientes con hiperglicemia al ingreso hospitalario de 13,65 con un intervalo de confianza al 95% de 11,27 a 17,71. Conclusión: La hiperglicemia al ingreso hospitalario se relaciona con la mortalidad en los pacientes no diabéticos con infarto agudo de miocardio. (p<0.05).
