Factores asociados de cefalea post-punción dural en gestantes sometidas a cesárea bajo anestesia regional

OBJETIVOS: Identificar los factores asociados de cefalea pos-punción dural (CPPD) en gestantes sometidas a cesárea bajo anestesia regional y la tasa de incidencia de CPPD.
METODOLOGIA: Estudio analítico, observacional y transversal, con seguimiento posoperatorio de 14 días vía telefónica de 134 gestantes entre 18 y 40 años sometidas a cesárea electiva bajo anestesia regional en el Hospital Belén de Trujillo.
RESULTADOS: La tasa de incidencia de CPPD fue 13,4 casos / 100 pacientes, 18 en la muestra estudiada. El 68.7% tenían menos de 30 años (p=0.3699); 48.5% con antecedente de cefalea antes o durante la gestación (p=0.2502); 27.6% más de una punción (p=02501) y 60.4% con tiempo de reposo menor a12 horas (p=0.1061).
CONCLUSIONES: la tasa de incidencia de CPPD de 13.4%, ésta no concuerda con lo encontrado en la literatura. Si bien en la muestra estudiada se evidencia una mayor frecuencia de CPPD en quienes tenían menos de 30 años, antecedente de cefalea, más de una punción y un tiempo de reposo menor a 12 horas de la anestesia, no se encontró asociación significativa con ninguno de los factores estudiados. OBJECTIVES: To identify the associated factors of post-dural puncture headache (PDPH) in pregnant women undergoing caesarean section under regional anesthesia and the incidence rate of PDPH.
METHODOLOGY: Analytical, observational and transversal study, with a 14 day postoperative follow-up by telephone of 134 pregnant women between 18 and 40 years who underwent elective caesarean section under regional anesthesia at Hospital Belen de Trujillo.
RESULTS: The incidence rate of PDPH was 13.4 cases / 100 patients, 18 in the sample studied. 68.7% were under 30 years old (p = 0.3699); 48.5% had a history of headache before or during pregnancy (p = 0.2502); 27.6% had more than one puncture (p = 02501) and 60.4% with rest time less than 12 hours (p = 0.1061).
CONCLUSIONS: The incidence rate of PDPH of 13.4% does not agree with what was found in the literature. Although the sample studied shows a greater frequency of CPPD in those who were under 30 years old, a history of headache, more than one puncture and a rest time less than 12 hours after anesthesia, no significant association was found with any of the factors studied.
