Objective: the aim of this study was to validate the technique of evidence of the retrieved angle (EAO) to diagnose mandibular second molars with C-shaped root canal.
Methods: A total of 198 dental records with a cone beam computed tomography (CBCT), obtained from the x-ray center Radiology Oral and Maxillo-Facial Tomography, Cuenca, Ecuador in the period March-August 2016, were analyzed in relation to the demographic characteristics and the presence of mandibular second molars with C-shaped root canals. Additionally, estimated the diagnostic accuracy of the EAO technique in panoramic reconstructions, which consisted of an angle whose vertex is placed in the center of the occlusal face and sides ended in the most prominent portion of the root apex, and their cut-off points were validated, using the CBCT as diagnostic gold standard.
Results: 116 (58.6%) of images were obtained from individuals of the female sex. The median age was 34 years (DQ=11). A total of 368 of the second mandibular molars tested, 33% had C-shaped root canal. Meanwhile, whereas 5.4° as the cut-off point, the technique obtained the following values in the validation test: sensitivity of 73,73% (CI 95%: 65,36; 82,09), specificity 87,83% (CI 95%: 83.38; 92.27), validity index of 83,05% (CI 95%: 78.96; 87.13), predictive positive value 75.65 (CI 95%: 67.37; 83.93), predictive negative value 86,70 (CI 95%: 82.12; 91.27) 0.62 Youden index (CI 95%: 0.53; 0.71), likelihood ratio positive 6.06 (CI 95%: 4.21; 8.71), 0.30 negative likelihood ratio (CI 95%: 0.22, 0.41) , area under the curve of 0.80 (SE: 0.02) (CI 95%: 0.76; 0.85).
Conclusions: These results allow to conclude that angles obtained with values ≤5,4° by the EAO technique are good, valid and effective for the diagnosis of this developmental anomaly in mandibular second molars.
