Resumen
The objective of this research was to design a pharmacotherapeutic conciliation protocol for patients admitted to the emergency service and with transfers to the Internal Medicine service at the Provincial General Hospital of Riobamba. During the investigation, a diagnosis was made through the methods, through the formulation and application of surveys to doctors, nurses and pharmaceutical biochemists in the aforementioned areas, where it was possible to gather information that showed the shortcomings on the information of the process of pharmacotherapeutic conciliation. For the conciliation process began with the review of 130 medical records in the emergency service, identifying that the most frequent medication error was incomplete prescription, followed by proceeding to reconcile 94 patients who were transferred from the emergency service to Internal Medicine service, of which in 39 patients the conciliation process was completed (stay and discharge) while, the remaining 55 patients could not complete the process (stay), identifying that the greatest discrepancy in the two cases was the therapeutic substitution due to lack of medication within the hospital classified in a justified discrepancy with a type A severity, that is, an error that has not been produced. With the training compiled, it was concluded in the development of the pharmacotherapeutic conciliation protocol under the NTE INEN 0 regulation, which established procedures and guidelines for the health personnel of the Energy and Internal Medicine services involved with the patient, in which it is intended to minimize the errors and discrepancies recorded in those areas.
