Resumen
The Pharmacotherapeutic Follow-up (PFU) was carried out on 20 hypertensive patients who attended the Pilot Plan Health Center outpatient service in La Concordia Canton, Santo Domingo de Los Tsáchilas Province during March - June 2019. For the subsequent detection, prevention, and resolution of problems related to medicines (PRM) and the negative results associated with medication (NRAM). The DÁDER method consisting of 7 phases was applied, blood pressure was measured once a week for three months, associated risk factors were assessed such as baseline glycemia, cholesterol, triglycerides, HDL, LDL. The Morisky Green Levine test was applied to determine the percentage of adherence to treatment. Identifying 46 PRM, the most relevant being 37.7% due to partial failure to comply with the given pattern, 26.0% due to inadequate preservation of the medication and 10.7% because the patient does not know that there is treatment for his / her problem of health, which generated 31 NRAM, of which 58.0% for effectiveness, 25.8% for need and 16.2% for safety. 65.2% Of the 100% of DRP detected were resolved through pharmaceutical interventions, 75.4% of them were Pharmacist-patient, and 24.6% Pharmacist-doctor. It has been a positive impact evidenced in the increase in 10% to 75% the adherence degree to treatment in patients, when applying t-student with a significance level of 0.05, a p = 0.0009 was obtained for systolic blood pressure (SBP), p = 0.003 for diastolic blood pressure (DPB), for glycemia a p = 0.01, in cholesterol p = 0.001, triglycerides p = 0.02, HDL p = 0.000 and LDL p = 9.004. As it has been valued less than 0.05, it is accepted the alternative hypothesis that states that there is a significant difference. All results show that SFT promotes the health of patients and improves their quality of life. It is recommended to continuously apply SFT in Health Centers to improve pharmaceutical services and clinical outcomes in patients, minimizing the morbidity rate.
