Resumen
The aim of the research was to apply pharmacotherapeutic follow-up to patients with diabetes and hypertension who received care at the Centro de Salud Quisapincha Tipo A, Tungurahua County, to do this, a qualitative descriptive method with a prospective cross-sectional design was used. A study of the current health status of the patients was carried out through the review of the health care registration platform (PRAS), where diabetic and hypertensive patients were identified, the Dader method, Morisky-Green test and the Goldberg test were applied in order to identify potential drug-related problems (DRPs) and negative results associated with medication (NRM). The sample was composed of 28 diabetic and hypertensive patients with other comorbidities associated with these pathologies during May-August 2021, in addition, the action plan for each patient belonging to the PFU was established and applied. The results obtained from DRPs were tabulated in Excel and subsequently analyzed, obtaining a 26% DRPs for necessity, 65% for effectiveness and 9% for safety. In the evaluation, NRM were found, the most frequent was quantitative ineffectiveness 69%, untreated health problems 25%, health problems due to necessary medication 9%, non-quantitative insecurity 7%, non-quantitative ineffectiveness 6% and quantitative insecurity 4%. It was concluded that it is necessary to carry out the PFU, due to the existence of interactions between drug-drug, drug-food and disordered eating habits, lately a socialization was carried out with the purpose of reducing possible adverse events and guarantee patient safety. It is recommended all patients with chronic non-communicable diseases or other pathologies be trained in the correct use of the medication.
