dc.description.abstractThe integration of Primary Health Care (PHC) with Health Surveillance becomes essential for the recognition of the territory, the population and the existing social dynamics. Thus, it is necessary to gather demographic, socioeconomic, political-cultural, epidemiological and health information, thus making it possible to identify and analyze people's problems, epidemiological profiles and health needs. Based on this purpose, the research aimed to carry out a health analysis of the territory of a municipality in the interior of Rio Grande do Sul through the perception of young students at a school. Therefore, the Participatory Rapid Estimation (ERP) methodology was used, using collective affective maps as an instrument for data collection to carry out the health analysis of the territory. Participated in this qualitative research 140 young students of the school, of both sexes, from the 5th to the 9th grade, including the EJA. In this way, we were able to formulate 20 collective affective maps, which young people could describe the potential and non-potential of the territory, through drawings and writing, including feelings. Being represented in the majority, violence, pollution, prejudice and drug addiction as factors that disadvantage the lives of these young people. The reorganization and integration of PHC with surveillance favor new relationships and work practices at all levels of care, thus bringing a continuous process, giving visibility to the conditions and health-disease needs in the territory, in the sense of helping to emerge a care of quality and, consequently, a strengthened SUS.
