Efetividade de uma intervenção participativa para promoção e apoio ao aleitamento materno por meio de rede social on-line: ensaio clínico randomizado

Importantes estratégias para a proteção, promoção e apoio à amamentação foram instituídas no Brasil, mas a realidade do país ainda é de persistência do desmame precoce. Essas estratégias necessitam serem mais efetivas no fortalecimento de uma rede de apoio a mulher que amamenta, especialmente no pós-parto. O apoio oferecido às mães neste período pode ser decisivo para a manutenção do aleitamento materno. O objetivo desta pesquisa foi avaliar a efetividade de uma intervenção participativa em rede social on-line para promoção e apoio ao aleitamento materno, desde a alta hospitalar da puérpera até o sexto mês seguinte. Realizou-se um ensaio clínico randomizado, no período de 2016-2017, que envolveu 251 nutrizes e seus respectivos filhos de um hospital universitário do Nordeste do Brasil, 123 alocadas no grupo intervenção e 128 no grupo controle. Após a alta hospitalar, o grupo intervenção foi acompanhado por meio de um grupo fechado em rede social on-line onde foram publicados cartazes semanais sobre temas relacionados à amamentação e estabelecido comunicação ativa com as mães. Os grupos foram entrevistados mensalmente por contato telefônico até a criança completar seis meses. O teste qui-quadrado de Pearson ou teste exato de Fisher, t-Student e Mann-Whitney foram utilizados para comparação entre os grupos das frequências, medias e medianas das variáveis. A duração do aleitamento materno foi calculada pela análise de sobrevivência e o efeito da intervenção foi estimado pelo modelo de regressão de Cox. As frequências de aleitamento materno foram maiores no grupo intervenção em todos os meses de acompanhamento, alcançando 84,2% no sexto mês versus 75,2% no grupo controle (p=0,087). O tempo médio de sua duração foi de 172 dias (IC95%:165,8-178,3) no grupo intervenção e de 159 dias (IC95%:150,1-168) no grupo controle (p=0,072). O risco proporcional de interrupção do aleitamento materno foi estimado em 0,57 (IC95%:0,32-1,03, p=0,065). Em relação à amamentação exclusiva, as suas frequências também foram maiores no grupo intervenção em todos os meses de acompanhamento, alcançando 33,3% no sexto mês versus 8,3% no grupo controle (p<0,0001). A mediana total do aleitamento materno exclusivo, de toda a amostra, foi de 117 dias (IC95%:101,5-132,5), sendo maior no grupo intervenção, 149 dias (IC95%:129,6-168,4), do que no grupo controle que foi de 86 dias (IC95%:64,9-107,1), (p<0,0001). O risco proporcional de interrupção precoce do aleitamento materno exclusivo foi 0,38 (IC95%:0,28-0,51, p<0,0001), demonstrando que a intervenção foi um fator de proteção que reduziu as taxas de interrupção precoce do aleitamento materno exclusivo nos primeiros seis meses em 62%, comparado com o grupo controle. Portanto, esta intervenção foi efetiva para promoção e apoio à amamentação, aumentando significantemente a duração e frequências do aleitamento materno exclusivo nos primeiros seis meses de vida. Sendo assim, a presente pesquisa propõe que os resultados encontrados colaborem para ressaltar uma metodologia inovadora de promoção e apoio ao aleitamento materno no período pós-natal que poderá ser utilizada na rotina dos serviços de saúde. Important strategies for the protection, promotion and support of breastfeeding were instituted in Brazil, but the reality of the country is still the persistence of early weaning. These strategies need to be more effective in strengthening a support network for women who breastfeed, especially in the postpartum period. The support offered to mothers in this period can be decisive for the maintenance of breastfeeding. The objective of this research was to evaluate the effectiveness of a participative intervention in online social network for promotion and support to breastfeeding, from hospital discharge of the puerperal until the sixth following month. Was carried a randomized clinical trial, in the period of 2016-2017, involving 251 breastfeeding women and their repective children from a university hospital in the Northeast of Brazil, 123 allocated in the intervention group and 128 in the control group. After hospital discharge, the intervention group was followed through a closed group in an online social network where weekly posters were published on topics related to breastfeeding and established active communication with mothers. The groups were interviewed monthly by telephone contact until the child completed six months. Pearson's chisquare test or Fisher's exact test, Student's t-test and Mann-Whitney test were used to compare the frequencies, means and medians of the variables. The duration of the breastfeeding was calculated by the survival analysis and the effect of the intervention was estimated by the Cox regression model. The breastfeeding frequencies were higher in the intervention group in all months of follow-up, reaching 84.2% in the sixth month versus 75.2% in the control group (p = 0.087). The average time of its duration was 172 days (95% CI: 165.8-178.3) in the intervention group and 159 days (95% CI: 150.1-168) in the control group (p = 0.072). The proportional risk of breastfeeding interruption was estimated at 0.57 (95% CI: 0.32-1.03, p = 0.065). In relation to the exclusive breastfeeding, its frequencies were also higher in the intervention group at all follow-up months, reaching 33.3% in the sixth month versus 8.3% in the control group (p <0.0001). The total median of exclusive breastfeeding for the entire sample was 117 days (95% CI: 101.5-132.5), the highest in the intervention group, 149 days (95% CI: 129.6-168.4), than in the control group, which was 86 days (95% CI: 64.9-107.1), (p <0.0001). The proportional risk of early interruption of exclusive breastfeeding was 0.38 (95% CI: 0.28-0.51, p <0.0001), demonstrating that intervention was a protective factor that reduced rates of early breastfeeding in the first six months by 62% compared to the control group. Therefore, this intervention was effective for breastfeeding promotion and support, significantly increasing the duration and frequency of exclusive breastfeeding in the first six months of life. Thus, the present research proposes that the results found collaborate to highlight an innovative methodology of promotion and support to breastfeeding in the postnatal period that can be used in routine health services.
