Os Determinantes sociais da saúde e o câncer de pênis: uma revisão integrativa

O Câncer de Pênis (CP) é uma neoplasia maligna rara que acomete homens na faixa etária a partir dos 50
anos e está associado às baixas condições socioeconômicas e culturais. Os determinantes sociais da saúde
podem ser caracterizados como não modificáveis e modificáveis. Os determinantes não modificáveis
normalmente estão intrínsecos a objetividade do sujeito, enquanto que os determinantes modificáveis estão
relacionados à subjetividade. No Brasil, o CP é um problema de saúde pública e representa 2% dos tumores
que mais acometem os homens, sendo as regiões Norte e Nordeste do país com maior número de casos. Este
estudo teve o objetivo de levantar os Determinantes Sociais da Saúde (DSS) associados ao câncer de pênis
através da publicação científica no período de agosto de 2008 a agosto de 2018. Reporta-se a uma revisão
integrativa que utilizou como critério de busca literária os artigos científicos disponibilizados completos
gratuitamente nos sites da PubMed e Scientific Eletronic Library Online (SCIELO) nos idiomas português,
inglês e espanhol através dos seguintes descritores: Câncer and pênis. O estudo permitiu realizar o
levantamento dos DSS conforme o modelo Dahlgren e Whitehead que estão associados ao câncer de pênis
como a idade, a raça, a etnia, o baixo nível de escolaridade, os hábitos de higiene, o desemprego, a renda
abaixo de um salário mínimo, a cultura machista, a fimose e o HPV/IST. Esse conhecimento favorece o
desenvolvimento de medidas de prevenção desta neoplasia CP preconizando o que determina a Política
Nacional de Atenção Integral à Saúde do Homem (PNAISH). Penile Cancer (CP) is a rare malignant neoplasm that affects men in the age group from the age of 50 and is
associated with low socioeconomic and cultural conditions. The social determinants of health can be
characterized as non-modifiable and modifiable. Non-modifiable determinants are usually intrinsic to the
objectivity of the subject, while the modifiable determinants are related to subjectivity. In Brazil, CP is a
public health problem and represents 2% of the tumors that affect men the most, being the North and
Northeast regions of the country with the highest number of cases. This study aimed to raise the Social
Determinants of Health (DSS) associated with penile cancer through scientific publication from August 2008
to August 2018. It is an integrative review that used as a criterion of literary search the scientific articles
available free of charge on the PubMed and Scientific Electronic Library Online (SCIELO) sites in
Portuguese, English and Spanish through the following descriptors: Cancer and penis. The study allowed the
study of the DSS according to the model Dahlgren and Whitehead that are associated with penile cancer such
as age, race, ethnicity, low level of schooling, hygiene habits, unemployment, income below one minimum
wage, macho culture, phimosis and HPV / STI. This knowledge favors the development of preventive
measures of this CP neoplasm, recommending what determines the National Policy of Integral Attention to
the Health of Man (PNAISH).
