Periodontitis is an inflammatory disease, mediated by the immune response of the host, and can be modified by systemic conditions, especially Diabetes Mellitus. Individuals with such a condition have a higher prevalence and severity of periodontal diseases, and adjuvant therapies may be indicated to basic periodontal treatment, such as the modified Intravascular Laser Irradiation of Blood (ILIB), which consists of a low-power laser coupled to a wristband at the height of the radial artery, radiating blood cells, promoting a decrease in the production of oxygen-free radicals, that are responsible for metabolic and tissue damage through the oxidative stress process, which can be evaluated through salivary biomarkers. OBJECTIVE: To evaluate biomarkers of salivary oxidative stress after the use of ILIB in patients with Periodontitis and patients with type 2 Diabetes Mellitus. METHODOLOGY: This is a blind and randomized clinical trial. The 17 patients were divided into a control group (CG), which received corono-root scraping and straightening (RACR), and test group (TG), which received RACR associated with ILIB. Saliva samples were collected, as well as evaluation of periodontal condition. The biomarkers of oxidative stress were evaluated by analyzing the levels of Superoxide Dismutase (SOD), Malondialdehyde (MDA), Reduced Glutathione (GSH) and Myeloperoxidase (MPO), through ELISA, baseline (T0) and after four months (T4). RESULTS: Regarding the intragroup analysis, there was a statistically significant difference (p<0.05) only in the MPO TG (p1=0.016). In the intergroup analysis, there was no statistical significance (p<0.05) for any biomarker. CONCLUSION: ILIB promoted an increase in MPO concentrations in the TG, but there was no improvement in salivary concentrations of SOD, MDA and GSH.
