Resumen
The Ministry of Health, through the Manual of Recommendations for the Control of
Tuberculosis in Brazil, warns that control measures be taken to interrupt the chain of
transmission of the disease. The objective of this work is to evaluate the strengths and
weaknesses of the implementation of administrative control, engineering and individual
protection measures in the control of Mycobacterium tuberculosis in the hospital service.
This is an integrative review of the literature with a synthesis of the studies published
between 2016 and 2020. Articles available in their entirety for free access, in English,
Spanish, Portuguese and French were included. A research protocol was constructed and
the guiding question was elaborated based on the strategy Problem / Intervention / Control
/ Outcome, where P (problem) corresponds to Tuberculosis, I (interventions) the TB
Infection Control Measures, C (control) does not apply to this proposal and O (outcome,
result) refers to infection control in the hospital service. The search for studies was carried
out in February 2021, in the databases US National Library of Medicine, ScienceDirect,
Cumulative Index to Nursing and Allied Health Literature and Literatura LatinoAmericana e do Caribe em Ciências da Saúde. The research strategy was given through
the crossing of the descriptors "Tuberculosis", "Infection Control" and "Hospitals". For
the analysis of the information, a Microsoft Excel spreadsheet was used, which allowed
the collection and subsequent categorization of the data. The information synthesis also
allowed the construction of the SWOT matrix (Strengths, Weakness, Opportunitys,
Threats) with the identification of strengths (strengths and opportunities), weaknesses
(weaknesses and threats), internal and external related to the control measures of the
tuberculosis infection. Of the 22 studies selected in the sample, (63.6%) were included in
the US National Library of Medicine, (50%) published in 2019, (95.46%) were in English.
There was a prevalence in the use of the quantitative approach in (95.46%) of the articles
and a predominance of the investigations carried out in the regions of Asia (40.91%) and
Africa (36.37%). The data analysis made it possible to identify the tuberculosis infection
control measures carried out in the hospital service, being (90.9%) administrative
measures. These results were separated by strengths, opportunities, weaknesses, and
threats. This data was organized in this way, since it was what the SWOT matrix
configuration required. The findings show that in 40.9% of the studies there are criteria
for the isolation of tuberculosis cases, 54.5% show bacteriological diagnosis, 31,8%
diagnostic imaging, 27.3% claim to provide tuberculosis treatment and 40.9% bring the
follow-up / identification / screening of respiratory symptomatic cases. Staff education /
training is mentioned in 50% of the surveys and 27.3% report the existence of a policy /
guideline / regulation for the control of tuberculosis infection. The control measures
identified were categorized into administrative, environmental and individual protection, with management measures cited in 90.9% of the studies. However, they should not be
applied in isolation to the detriment of others, but together to improve the effectiveness
of actions. When analyzing the matrix, 13 weaknesses and 24 strengths. Although weaker
points have been identified less frequently, these negative aspects cannot be neglected,
with a view to developing an action plan that aims to overcome the weaknesses presented
by the matrix.
