The Family Health Strategy (FHS) was created to be instrumental in the reorganization of primary health care in Brazil. Universality, equity, integrity, commitment, continuity, accountability, coordination of care are some of the principles of the Unified Health System (SUS). We can see the absence of a facilitator, in health services oral, for the execution of these principles. This paper aims to propose this instrument, shaped like a book that will accompany the oral health from birth to 10 years of age in the FHS. Monitoring childhood becomes important, because this time of life to create habits that will last a lifetime. The booklet national child health is an important tool for monitoring children's health, but with little emphasis on oral health. We conducted a literature review, considering the period from 1994 to 2011, about the importance of early intervention and long-term monitoring. Topics such as feeding, teething, oral hygiene of infants and children, nutrition, accidents and antibiotics are covered in this book. The proposal to create a book of oral health for children up to 10 years, is reflected as an instrument for the socialization of information, be a bridge for access to oral health services, on condition monitoring, and effective co responsibility of parents in oral health care of their children. Further studies will be needed for validation and adjustments of this instrument.
