Resumen
Diabetes mellitus (DM) is a syndrome of multiple etiologies, resulting from the lack of insulin and/or inability of insulin adequately exercise their actions, characterized by chronic hyperglycemia and changes in the metabolism of carbohydrates, lipids and proteins. The natural history of DM is marked by the appearance of chronic complications, classified as microvascular and macrovascular complications (retinopathy, nephropathy, neuropathy, oença coronary artery disease, cerebrovascular disease and peripheral vascular). On the occasion of the situational diagnosis made in the PSF Celso Diana, Minas Gerais State, we observed a high prevalence of patients with DM. Thus, this study aimed to develop an intervention plan that enables the modification of risk factors for complications among patients with Diabetes Mellitus in the PSF Celso Diana, Santa Luzia, Minas Gerais. For the development of the Plan of Intervention was used the Method of Strategic Planning Situational - PES (FIELDS; FARIA; SANTOS, 2010), in addition to bibliographic research on the topic in journals indexed to the Virtual Health Library (VHL), System to Search for information on health of the U.S. National Library of Medicine (NLM - Pub Med), Virtual Library of Universidade Federal de Minas Gerais and Scientific Electronic Library Online (SciELO). It is hoped this work a satisfactory control of the disease and reduction in new cases of DM in the area of scope, reducing the number of hospital admissions, decompensations and complications that this disease causes the population of the territory.
