This paper presents the incidence of Aids and mortality among outreached Injecting Drug Users (IDU) of the AjUDE-Brasil II Project. From a cross-sectional survey, 478 IDU were interviewed in three Brazilian cities named Porto Alegre (RS), São José do Rio Preto (SP)and Itajaí (SC). This cohort was followed up in the Brazilian surveillance database for Aids and death during 2000 and 2001. Aids incidence was 1.1 cases per 100 persons-years and the mortality rate was 2.8 deaths per 100 persons-years. Aids cases occurred only among IDU who ever have reported syringe sharing. Female gender (RR = 5.30), homelesscondition (RR = 6.16) and report of previous sexual intercourse with same-sex partner (RR = 6.21) were associated with Aids. Deaths occurred only among males. Being homeless (RR = 3.00), without income (RR = 2.65), HIV seropositive (RR = 4.52) and never beingincarcerated (RR = 3.71) were also associated with death. These findings support evidences that gender and socio-economical conditions are both determinants for morbidity and mortality of Brazilian IDU
