Systemic Hypertension is considered one of the major modifiable risk factors and represents a major public health problem. This research aimed to develop an intervention proposal for the efficient control of high blood pressure and cardiovascular risk in population served by the team Belo vouchers Rancho Alegre Health Program of the City Divinópolis Family. The preparation of this proposal consisted of three stages: the situational diagnosis in health, literature review and the preparation of the action plan. The action plan was based on the method of Situational Strategic Planning (PES) and the review of the literature was made in the Virtual Library of Health, with the key words: hypertension, complications and prevention. . Were also surveyed the Ministry of Health Programs The proposed operations were: Deploy the working line and provide greater understanding of the health team and raise the population's level of information on hypertension and care for cardiovascular risk; expand individual and collective knowledge to improve, modify and promote customs, habits and healthy lifestyles and improve the structure of basic health services. It is believed that the proposed intervention for ESF Belo Vale- Rancho Alegre enable more effective monitoring to patients with systemic hypertension
