The patellofemoral joint is influenced by the hip mechanics and the ankle-foot complex. The Patellofemoral Pain Syndrome (PFPS) is one of the most frequent pathology in physiotherapeutic practice. Due the influence of the hip on the patellofemoral joint mechanics, the relationship between weakness of the hip muscles and PFPS have been studied. The aim of this systematic review was to investigate whether the strengthening of hip muscles increases strength and level of activity, and reduces the pain of patients with PFPS. For this, searches for randomized, quasi-randomized or controlled clinical trials were conducted in the databases Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, MEDLINE, PsycINFO e PEDro. After an accurate analysis, 12 studies were selected. The studies were not conclusive for strength, but they demonstrated that hip strengthening is effective in reduce pain. In general, the increase of the level of activity was found after to concentrate on the strengthening of hip muscles. Strengthen the hip muscles seems to be linked to early recovery for the patient when compared to the conventional treatment exclusively focused on knee muscles
