The present article tried to elucidate, by means of a literature review, the relationship between the smoking habit and the success of osseointegrated implant therapy. It is widely known that smoking impair dental implants. Although the mechanisms are not completely clarified, this article tried to bring in the understanding of the harmful effects of tobacco related to peri-implantar tissues, especially to the bone tissue, and whether there is any significant risk of dental implant loss due to active cigarette smoking. According to literature, smoking is understood as a significant risk factor for osseointegrable dental implant loss. The purpose of the present study was also to investigate the relationship between smoking and bone quality, the relationship between smoking and peri-implantar bone loss and, additionally, whether the modification of implant surfaces, also called surface treatment, could cancel the negative effects of smoking on osseointegration of dental implants. It was concluded that smoking worsens bone quality, causing the increase of peri-implantar bone loss in smokers and that, despite the controversial results, it can not be inferred that the modification of dental implant surfaces increases the success rates in patients who smoke.
